Job Description

Would you like to join our progressive, ward based and patient focused Pharmacy service?

As we continue to develop our clinical services we have opportunities for highly motivated and enthusiastic pharmacists to join our friendly team. This post offers you the opportunity to progress your career in an innovative and supportive environment.

We encourage applications from trainee pharmacists due to register after completing the GPhC assessment in August 2023 and qualified pharmacists from other sectors who are interested in expanding their practice into hospital.

A relocation payment of up to £7,700 is available for those relocating to the area.

The successful applicant will join our friendly team of rotational pharmacists. We provide rotations through medicine (cardiology, respiratory, endocrinology, gastroenterology, stroke and frailty), surgery, acute admissions,paediatrics,community hospitalsand mentalhealth.

As Somerset NHS Foundation TrustandYeovil District Hospital NHS Foundation Trustmerge in 2023 we will be providing clinical pharmacy services to two acute hospitals, specialist mental health units and community hospitals across Somerset. This is a great opportunity to develop clinical skills in a wide variety of settings and to work alongside pharmacists specialising in these areas.

You will be supported by our team of senior pharmacists (including independent prescribers) and by accredited medicines optimisation technicians and assistants in our ward based Medicines Optimisation Teams.

Your training opportunities will be further supported through the opportunity to undertake a fully funded Clinical Pharmacy Diploma (including independent prescribing qualification) with The University of Bath.

Somerset NHS Foundation Trust provides acute health services for its local population of around 340,000 people and provides specialist services for a larger population of around 544,000 people. The hospital also provides emergency vascular services for Somerset and some parts of Dorset covering a population of around 800,000 people.

To act as a clinical pharmacist within the Trust providing high quality and professional input into the delivery of clinical and operational pharmacy services

To work in all sections of the pharmacy service on a rotational basis under the direction of the relevant lead pharmacy staff. These areas include dispensary, medicines information, medical and surgical wards, and chemotherapy production.

Support safe, effective and efficient medicines and medicines information provision for patients and/ or carers, medical and nursing staff and other healthcare professionals throughout the whole organisation. This will include directing and checking the dispensing or issuing of medicines from all areas of the pharmacy department and screening for drug incompatibilities, interactions and prescribing errors.

To clinically assess patients' medication charts on a daily basis on allocated wards, including medicines reconciliation, to support safe and effective medicine optimisation.

Provide general high quality clinical pharmacy services as a member of the clinical pharmacy team meeting the needs of patients and healthcare professionals within the trust.

Support clinical governance processes, by incident reporting, following Trust and departmental guidelines, policies and procedures.

Support the delivery and development of new clinical pharmacy services to facilitate pharmaceutical care.

To actively participate in mandatory Continuing Personal Development (CPD) Programme as required by the General Pharmaceutical Council (GPhC)

Participate in continuing education through appropriate training schemes, clinical diploma where appropriate and self-directed learning, in line with clinical governance requirements.

To participate in the on-call, evening, weekend and bank holiday rotas.
