MPFT is a health promoting Trust supporting our patients to achieve the best possible health and wellbeing. We believe that alongside providing high quality clinical care we should support our patients with the broader non-medical and holistic needs that impact on their mental and physical wellbeing.
Enabling patients to be active participants in their care and to take positive steps through better lifestyle choices, improving their health knowledge, enabling them manage their own health conditions and access the services and help they need to support their social wellbeing.
Thesenew roles work with a number of MPFT services as part of wider multi-disciplinary team under the supervision of clinical, public health and social care colleagues. The role is varied, and may include elements of supported self-management, patient education, case management and facilitating group consultations all under the supervision of a qualified practitioner.
MPFT are seeking people from a diverse range of backgrounds with highly developed people skills and a strong desire to improve people's lives. You will have experience of motivating and supporting people to make positive behavioural changes. All successful candidates will be supported through a comprehensive programme of training and development, including undertaking national accredited health coaching training.
The post holders will work under the supervision of clinical, public health and social care colleagues, to ensure the role delivers the best possible outcomes for our patients. The role is varied, and may include elements of supported self-management education, case management and facilitating group consultations all under the supervision of a qualified practitioner Post holder will take an approach that is non-judgemental, based on strong communication and negotiation skills. Post holder will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions. As a whole your role and skills will support and encourage living well, the prevention of developing further illness, and/or the deterioration of existing long term conditions.
