An exciting opportunity has arisen for a social worker to come and join us in the Leeds Cancer Centre at Leeds Teaching Hospitals Trust. You will be expected to work closely with health colleagues and will work in an integrated way to advise and support patients and their families.
You should have up-to-date professional expertise and experience in case management, take responsibility for assessments, reviews and support planning, working with a multi-disciplinary team and work within the parameters of the Care Act 2014 and Mental Capacity Act 2005 as well as have an in-depth current knowledge of all other relevant legislation, including NHS Continuing Healthcare.
The role will be based with the Oncology Clinical Service Unit (CSU) discharge team.
