Job Description
Working alongside clinical and social care teams within the Maidstone and Tunbridge Wells acute trust to embed a Social Prescribing service for the benefit of patients. A holistic assessment of their wellbeing and development of a support plan maximising community assets and building self-care skills.
Link Worker (Hospital Focus) –
Hours: 37.5 hours Monday to Friday
Annual Leave: 30 days plus bank holidays

Responsible to: Hospital Projects Manager
Based: Working West Kent wide, including attending Tunbridge Wells and or Maidstone Hospital (petrol expenses paid at 45p per mile.) Office at Turkey Mill, Maidstone.
Focusing on the discharge from hospital and attendance at A and E of patients who may need help, advice and or support when home from hospital. Working to personalised care and the Esther model, listening, and giving patients time to identify “What matters to me”. Link workers will support patients for up to 6 weeks after attendance at hospital and seamlessly refer those who need longer term support to the relevant services.
Please note this role includes home visiting people and attending Maidstone or Tunbridge Wells Hospitals, therefore a full driving licence and access to a vehicle is a requirement.
This role also includes providing regulated activity with adults and employment is therefore subject to relevant checks with Disclosure and Barring service.
We are looking for someone with excellent communication skills, competent and experienced with IT and databases, outgoing, energetic, with a can do and problem-solving approach to improve the wellbeing of others. Positive, resilient, able to support and empower people with complex needs with care and compassion.
