About the role:
We are looking for a full time Referral Taker who enjoys working as part of a team to join our Hospital Discharge and Assessment Team.
The Referral Taker is the first point of contact in the team; they are responsible for triaging information provided, liaising with partner agencies and taking comprehensive, holistic referrals.

About you:
The successful candidate will have excellent communication skills and the ability to remain calm and focused in a busy environment, maintaining a positive team working approach. The role needs a holistic approach and an ability to create accurate referrals of appropriate urgency for relevant teams or for signposting and directing to other agencies.
You will have strong organisational skills with experience of responding flexibly to changing priorities. Experience of working in a customer facing environment is essential, as are excellent IT skills. Knowledge of the Liquid Logic (LAS) database would be beneficial but full training will be given.
