Supported Living Care Worker

Job Description

Responsibilities:

• Takes pride in working with vulnerable people and takes safeguarding responsibilities seriously
• Work with a team of care workers and support staff to coordinate the delivery of services to a client's home
• Preferably has successfully completed NVQ Level 2 in Adult Social Care (or equivalent)
• Achieves all Health & Safety standards
• Identify needs that are not addressed by existing services and advocate for their resolution.
• Communicate with families, caregivers, and service providers to ensure that care plans are implemented and individualized based on the client's needs.
• Share feedback with the care team to address issues and make recommendations for continuous quality improvement.
• Review service files to ensure that all records are complete and accurate.
• Effective decision-making and judgment
