Job Description

We are looking for a clinician (from medical, nursing or AHP background) with a strong interest and experience in population health approaches to help transform healthcare for the future. The Health Inequality Fellow will work closely with clinicians to provide support, training and advice to develop key areas for investigation and use population health data to define their key questions and shape local interventions on the ground. We are therefore looking for candidates with a keen interest in developing their population health competencies to inform key-decision making and improve health outcomes for the population.

PLEASE NOTE: This post will be available on NHS payscale and the medical and dental payscale.

Fellow(s) will be based, for up to three days a week, with the Lewisham Population Health and Care programme team and having prior knowledge of Lewisham’s landscape and its demographics would be a benefit. They will be supported by a suitable senior manager or population health advisor during their placement.

Key areas of work that we expect the Fellow to focus on will include:

1. To agree the rationale and drivers for exploring population health ideas/ problem statements
2. To establish how our information system (HealtheIntent) could support local work and translate data into actionable insights
3. To engage with clinical leads to set up and lead any working groups required to drive forward and manage the projects
4. To agree the characteristics and activity to case-find patients
5. To agree what the expected outcomes would be for projects
6. To support the development of a dashboard (or other tools if required) for end-users to support case-finding based on the above criteria
7. To agree the relevant interventions with colleagues based on the areas where the data suggests there is room for improvement
8. Facilitate MDT discussions as required
9. To agree how to realise benefits and monitor those benefits
10. To agree how they would embed the work as BAU
11. To present project(s) as and when requested
12. To consider wider dissemination of their project(s) through write-ups or sharing with other partners

Lewisham and Greenwich NHS Trust is a community focused provider of local and acute care, providing high quality services to over one million people living across the London boroughs of Lewisham, Greenwich and Bexley. With a turnover of around £700 million, we provide services at Queen Elizabeth Hospital in Woolwich, University Hospital Lewisham and a range of community settings in Lewisham. We also provide some services at Queen Mary’s Hospital in Sidcup.

As an organisation, we’ve made significant improvements for patients and staff in the last two and-a-half years. This is reflected in our most recent Care Quality Commission (CQC) inspection (from February 2020), which found improvements across the organisation and gave us an improved rating of “Good” in the Well-Led and Effective domains, in addition to the rating of “Good” that we had already achieved in the Caring domain. Our staff culture change programme has been key to the Trust’s improvement journey, as we’ve stabilised our workforce by reducing vacancy rates and improving staff retention. We’ve also significantly increased the representation of minority ethnic leaders across our clinical and corporate teams.
The post-holder will work closely with Lewisham Population Health and Care team, as well as wider stakeholders to develop projects. Organisations include:
