Job Description

An exciting opportunity has arisen within the Sheffield GP Collaborative.

The Sheffield GP Collaborative (GPC) provides an urgent out of hour’s primary care service for the city of Sheffield. This includes providing clinical triage for patients referred by NHS 111, face to face clinical assessment at the GP collaborative and when required home visits for the housebound and those at the end of life.

The purpose of the role is to provide a high-quality service for patients and healthcare professional using the Out of Hours GP Service. To achieve and maintain high standards, developing effective working relationships with all service users. To provide support to GPs, Advanced Care practitioners, colleagues, patients and other health professionals as required.

The role involves working unsociable hours including overnight shifts, weekends and bank holidays.

Please note this advertisement is for a both permanent (18 hours) post and a fixed term (30 hours) opportunity.

The 30 hour post is advertised on a fixed term contract basis for the maximum period of 12 months. Where there is potential, the secondment/fixed term contract may finish early, or in the first instance, the secondment/fixed term contract may be extended due to the internal/external secondment of the substantive post holder. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and secondments in relation to the substantive post holder.
