To be responsible for designated aspects of nursing care for a defined
group of patients under the direction of a registered nurse.
Perform more complex elements of patient assessment from admission
through to discharge. Plan and deliver high standards of care to individuals and groups of patients. (Under indirect supervision from the registered nurse).
To understand and adhere to all relevant trust policies and procedures. To prioritize care as appropriate.
To demonstrate good communication skills when dealing with patients,
relatives and members of the multidisciplinary team utilising sensitivity, empathy and ability to manage conflict.
To immediately escalate a clinical or non-clinical incident to the registered nurse.
To understand the principles of the Friends and Family Test and patient
feedback and how it is used to improve and maintain an excellent patient
experience.
Contribute to effective and timely discharge planning.
To keep nursing staff fully informed and to seek advice and support when
indicated.
To demonstrate initiative and the ability to recognise/identify changes in a
patient’s condition/treatment and report promptly to the registered nurse and medical staff as appropriate.
To liaise with relevant healthcare professionals/departments e.g. dietician,
physiotherapist, equipment library and to keep registered designated nurse fully informed. To perform, accurately record and interpret patient observations -temperature, pulse, blood pressure, food and fluid charts, blood glucose monitoring and urinalysis.
To perform, understand and report patient risk assessments e.g. Nutritional assessment scores, Falls Assessment, Moving and Handling, Pressure Scoring Tool Assessment and Bedside Rails Assessment.
To perform additional tasks (as directed by nursing staff) following
completion of relevant competencies/training.
Act in accordance with the Code of Conduct for Healthcare Support Workers in England.
