Job Description
Hounslow and Richmond Community Healthcare (HRCH) NHS Trust in partnership with Hounslow GP Consortium Ltd has exciting opportunities for a number ofclinical pharmaciststo work in our Primary Care Networks - Chiswick, Brentworth&Isleworth, Feltham&Bedfont, Great West Road or Hounslow Health PCNs.
You will be an integral part of a wider team of PCN clinical pharmacists and PCN Pharmacy technicians working under the mentorship of Senior Clinical Pharmacists. You will develop through team clinical supervision sessions and benefit from direct mentoring with experienced GPs and Clinical Directors.
- Work part of a multi-disciplinary team in a patient-facing role
- Conduct structured medication reviews to proactively manage patients withcomplex polypharmacy
- Provide clinical medicines expertise to both patients and general practice team
- Improve prescribing practices across the Primary Care Network
- Improve service and quality through mechanisms such as audits and PDSAcycles
- Interface with community and hospital colleagues to help resolve medication relatedissues
- Assist with delivery of enhanced health in care homes initiatives

It is a requirement of the role to be registered with the General Pharmaceutical Council. Primary care experience would be an advantage, however, is not essential. Flexibility, enthusiasm and good clinical knowledge are essential. Successful candidates will be supported in terms of clinical learning and professional development through access to a GP clinical supervisor, other clinical pharmacists and enrolment onto a formal education pathway delivered through the Centre of Postgraduate Pharmacy Education (if not already completed).We have a strong community of practice amongst our growing pharmacy team, you will benefit from peer to peer support and encouragement to continue your career development towards advanced practice and beyond.
