The Deben Health Group PCN is looking for two new members for our growing patient coordinator team. This is a varied role, with a lot of scope for development, and would interest anyone with an administrative background looking for a new and challenging role within primary care.
You will act as an important tool to help coordinate the surgery’s patient case load and the work needed to fulfil the PCN's DES specific contracts. Patient coordinators will be expected to support their linked surgery with tasks relevant to their role and the needs of the surgery.
Key Responsibilities
- To assist the doctors, clinical pharmacists and other clinicians in the provision of a high quality service.
- Be the point of liaison for patients and interface with all health and social care professionals keeping everyone informed and updated.
- Organise MDT meetings
- Receive discharge summaries from hospital and complete care calls
- Contacting secondary care to chase referrals
- Working closely with care homes and completing patient personalised support care plans

Training
Candidates should expect to be enrolled with the Personalised Care Institute and undertake both mandatory and on the job training. The PCN will support and encourage post holders to undertake any training, seminars or workshops which are relevant to the role.

Travel between Rendlesham Surgery and Framfield House Surgery (Woodbridge) is expected.
It is desirable that candidates have worked in primary or secondary care, are adaptable in their working approach, have experience of talking to patients or have worked in a customer facing environment and can demonstrate solid administrative and organisational skills.
An understanding of SystmOne is desirable, but not essential, as training will be provided for the right candidate(s).

The successful candidate would join our Team of Patient Coordinators, Mental Health Practitioners, Pharmacy Technicians, Clinical Pharmacists, Data Administrators, Trainee Nursing Associates and First Contact Physiotherapists.
