We are a dynamic, forward thinking PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients and reducing health inequalities.
We are looking to expand our team by recruiting a team of GP Assistants to work alongside our practices and PCN staff. Our PCN staff team currently include clinical pharmacists, pharmacy technicians, physician associates, social prescribers, health and wellbeing coaches, care co-ordinators, nurse associates, occupational therapists, mental health nurses, and paramedics.
Initially we are looking to create two different teams one with a primarily administrative function with the opportunity to develop clinical skills and knowledge, and one team that is primarily clinical supporting housebound and care homes patients as well as the practices. As these are new roles the duties and responsibilities will evolve and therefore the successful candidate will need to be able to adapt to the needs of the PCN.

Main duties of the job
The Administrative team will be responsible for:
Summarising medical records
Sorting all clinical post and prioritising for the GP in terms of actions / signposting
Coding clinical information from correspondence
Arranging appointments, referrals, tests and follow up appointments of patients
Medication changes from letters
Data quality - improving diagnosis rates and increasing prevalence
Support reception staff, providing cover
There will be opportunities to develop clinical skills and knowledge.
The Clinical team will be responsible for:
Preparing patients before seeing the GP, taking a brief history and basic readings
Completing basic (non-opinion) forms and core elements of some forms for the GP to approve and sign such as insurance forms, mortgage, benefits agency forms etc
Accurately read coding data on SystmOne into the patient record
Dipping urine, taking blood pressure, ECGs & phlebotomy
Helping the GP liaise with outside agencies e.g. arrange admission
Support with immunisations/wound care
Supporting clinicians for weekly ward rounds in care homes
Completing basic observations and LTC pre-assessments for care home and housebound patients
Personalised Care Plans within the clinical systems with correct coding
Communicate with the patients family/carers on behalf of the GP
Support with the delivery of immunisations
Ultimately these teams may merge.

We are looking to recruit a number of people to these roles to create two new teams. The successful applicants will work within the PCN, working closely with Practices alongside existing clinical and administrative teams to deliver excellent patient care. This role provides fantastic opportunities for career development.
You will develop and maintain knowledge, skills and competencies related to the role of GP Assistant.
You will also have experience of working in health, social care, or other support roles that include direct contact with people, families, and carers. To be eligible for this role you must:
- Have a minimum of GCSE Maths and English at grade 9 to 4 (A to C) or Functional Skills Level 2 in Maths and English
- Have experience working as a team
- Have experience in Primary Care (preferably)

- Main Duties and Responsibilities:
- Deliver high quality, compassionate care under the direction of a GP / Practice Manager / PCN Business Manager

- The Clinical team will be responsible for:
- Preparing patients prior to going in to see the GP, taking a brief history and basic readings in readiness for the GP appointment.
- Completing basic (non-opinion) forms and core elements of some forms for the GP to approve and sign such as insurance forms, mortgage, benefits agency forms etc
- Accurately read coding data on SystmOne into the patient record
- Input data into the patients healthcare records as necessary
- Input and extract data as required to support practice reports

- Help on running searches as discussed, recalling patients for enhanced service, IIF, and QOF work etc
- Dipping urine, taking blood pressure, ECGs & phlebotomy
- Helping the GP liaise with outside agencies e.g. getting an on call doctor on the phone to ask advice or arrange admission while the GP can continue with their consultation(s)
- Support the GP with immunisations/wound care
- Supporting the GP when completed the weekly ward rounds in the Care Home, including completing basic observations
- Completing basic observations and LTC pre-assessments for care home and housebound patients
- Awareness of contractual responsibilities (LES and DES requirements) and ensuring these are met for all the patients within the care home setting
- Prepare list of patients for the GP ensuring that all pre work for the consultation is completed before hand e.g. blood results, urine samples.
- Completing referral letters / forms
- Ability to write Personalised Care Plans within the clinical systems with correct coding
- Liaising with other Health Care Professionals both internally and externally to the PCN to ensure the whole needs of the patient are met
