We are looking for someone with passion, empathy, and experience to deliver support as part of the Woodspring Recovery Navigation team in North Somerset; including Clevedon, Portishead, Long Ashton and surrounding areas. We are looking for someone to work as part of a team to ensure people with moderate to serious mental health needs are supported to connect to their community, to build resilience and to improve their wellbeing.
Working collaboratively with the client, GP and other health and care professionals, you will provide practical and emotional support that is tailored to the individual. This may include things like attending an important appointment with them, helping them fill out a form or giving them information about other services.
You’ll be a good communicator, have experience of working with people with long term mental health and complex needs and carrying out needs and risks assessments. You will need to take a holistic view, with a recovery and psychologically informed approach. You’ll use your experience in the care and support sector to contribute to the delivery of this valuable service.
