ABBEY MEDICAL PRACTICE

JOB DESCRIPTION

JOB TITLE: Clinical Pharmacist

HOURS: 24 hours per week over 3 days

REPORTS TO: The GP Partners – clinical and professional activities

The Business & The Practice Manager – administrative and

non-clinical activities

BASED: Abbey Medical Practice, Abbey Lane, Evesham, WR11 4BS

JOB PURPOSE

CORE RESPONSIBILITIES

- Frail elderly

- Polypharmacy

- Renal impairment

- Hepatic impairment

- Asthma, diabetes, hypertension and Rheumatoid and DMARD drug monitoring

- Substance misuse

- Patients on high-risk medicines

- STOPP/START identified patients

- Revolving door Hospital admissions
