Title: Mental Health Care Navigator

Location: HMP Humber

Salary: £21,703.54 to £23,906.01 Per Anum

Rethink Mental Illness are excited to be setting up a new IAPT service in HMP Hull in partnership with Tees, Esk and Wear Valley NHS Foundation Trust and Humber Teaching NHS Foundation Trust.

This service will see the implementation of a stepped care model into HMP Hull, allowing residents to access evidence based, effective care comparative to what is offer in the community.

Care Navigators aim to provide a seamless package of support to individuals with mental health needs who are leaving the prison and returning to the community, reconnecting individuals with health services to reduce health inequalities and in turn reduce reoffending. The Care Navigators are embedded in integrated mental health teams and community settings, providing 1:1 and group assessment and support in a variety of settings and hold highly autonomous roles.

To be eligible to apply for this role you must be eligible to work in the UK, and provide proof of this (i.e., a passport/visa). You will also be subject to National Prison Vetting Security Clearance.

How you will make a difference:

You will provide Care Navigation Services to individuals being released from the HMP Hull or HMP Humber, supporting people both pre and post release in line with service guidelines, working alongside other agencies to improve health outcomes and reduce reoffending

You will provide support to those being released from court, and provide assistance with these processes by completing an initial screening and care mapping assessments with each service user and use service documents to plan their care and release, utilizing supervision to ensure a high quality of care provision.

You will develop tailored models of support for patients through a variety of modalities, mediums and with varying intensity to promote independent self-management and engagement with statutory and non-statutory services

Clinical work is carried out in both the prison and community settings, so access to transport for work purposes is essential.

What we are looking for:

You will:

  • be educated to NVQ Level 3 or equivalent with qualifications or training relevant to the Care Navigation role, e.g., Health and Social Care
  • have relevant experience of supporting vulnerable individuals to identify, set and meet their own goals using 1:1 and/or group interventions
  • have a working knowledge of health, debts, social care, housing, court systems, mental health, criminal justice, and substance misuses
  • have experience of working to support individuals using structured, time limited support and am confident with this way of working
  • be passionate and motivated to support individuals leaving the criminal justice system through provision of a high quality, professional level of care, recognizing and respecting individual differences and promoting diversity
  • value the unique life journey each individual has been on, and embrace the challenges this can bring
