JOB BRIEF
We are seeking care support worker who will enable service users to lead as active and independent lives as possible by providing holistic individualised outcome focused care to service users . And to also promote choice to customers in their daily living by providing care which meets Care Quality Commissions (CQC) fundamental standards and supports happy living for the years ahead including mental and physical wellbeing .
Responsibilities
- To provide person centred care and/or support for service users to the required standard in areas including but not limited to personal care; maximising physical and mental wellbeing, personal safety, participating in the local community; household living skills and money management; promoting positive participation and inclusion at all times. To be able to work positively with vulnerable people including young people and those with disabilities. To support people to reach to reach their goals and aspirations.
- Maintain and complete accurate documentation of all care and support undertaken. Recognise, document and report any changes in the care plan, and participate in customer reviews as required.
- To develop support plans based on risk assessment and risk management as required and to support service users to meet their needs, goals and aspirations and support agreed outcomes within the support plan
- To take appropriate action to ensure compliance with all statutory requirements, performance standards and client's policies and practices.
- Comply with clients procedures and standards at all times
Requirements
- NVQ 2 or willing to work towards Diploma in Health and Social Care
- Ability to read, understand and complete / contribute to care plans

Salary: £11.05-£22.10 per hour
