The ICB is looking for a highly motivated practitioner with excellent assessment & communication skills to join our busy but friendly Continuing Healthcare team. This is an excellent opportunity to work alongside highly skilled nursing professionals recognised for their broad spectrum of experience
As a CHC Nurse Assessor, you will be part of a strong, supportive high performing team with a focus on quality. You will be integral to making recommendations for CHC eligibility as well as supporting the implementation & delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care
The role will be varied with no two days the same, using your knowledge & clinical skills you can expect to be involved in many aspects of the teams functions, including, clinical assessments, co-ordinating the process of sourcing appropriate care provision, case management, liaising with key stakeholders to ensure timely & safe patient transfers
Importantly, you will develop & influence the quality of care that is provided to patients to improve patient experience of care
Whilst it is highly desirable for you to have prior experience of NHS Continuing Healthcare this is not essential. The successful candidate will be determined through demonstrating the right aptitudes, a full induction with ongoing training & support will be provided
It is important to note that you will need the ability to travel to a variety of locations to undertake this post
