This is an exciting opportunity to join the Complex Lives Team, a unique, friendly and positive County Wide Living Well team.
As a valued member of this small team, you will be supporting people aged 18-65 with care and support needs, within the Learning Disability and/or Autism Programme, with a focus on supporting people (who are subject to the Mental Health Act 1983) to be safely discharged from long-term hospital admissions.
You will be passionate about supporting vulnerable people and delivering increased outcomes for this group. You will need to have a good understanding and experience of the application of the Care Act 2014, Mental Health Act 1983, Mental Capacity Act 2005 and will bring a commitment to the principles of strength’s-based practice, promoting independence and wellbeing.
We welcome applications from Social Workers who have no previous experience of the Complex Lives Team as support and training will be provided and the team is a very supportive and experienced service.
As an ideal candidate, you will be:

Registered with Social Work England
- 
Experienced Social Worker
- 
Have effective communication, negotiation, organisational and IT skills
- 
Be committed to interprofessional collaborations with partner agencies
- 
Be able to work autonomously and have a high degree of resilience
- 
Able to drive and holds UK Driving License
You will be expected to undertake a range of Social Work tasks including, Care Act Assessments, Support Planning, robust Risk Management work, and the Commissioning of services. In addition, you will work in partnership with a variety of health partners to deliver positive outcomes for the people we serve.
