Family Hub Practitioner (Universal Offer – Nursery Nurses x5)

This is an exciting opportunity to join at the start of the new Family Hub Programme, we expect this role to develop over the life of the programme. We welcome your existing knowledge, skills and experience of working as a nursery nurse in shaping the universal offer and integration of services to ensure all children get the best start in life!

Main duties of the job

You will join one of five Dudley Family Hub Centres/Health Visiting Teams that will offer an open universal front door to families providing a welcoming experience and support that identifies family strengths and community to help families meet their potential. You will offer support across all services within the Family Hub/Health Visiting teams you will be expected to embrace the 3 principles of Family Hubs – Access, Connection and Relationships when working with colleagues and with families. You will facilitate groups including evidence-based programmes such as Triple P and Henry, well baby clinic, development clinics as well as open access stay and play sessions and other peer to peer sessions. You will provide outreach to help overcome any barriers for families in accessing and engaging with the Family Hubs. You will get to know the community assets within your area so you can help families to navigate themselves to the support that they want including virtual/digital offers. You will develop extensive knowledge of the integrated offer and signpost to services appropriate to the needs of the families across the age range. You will engage in an ongoing “continuous conversation” with families in order to listen and understand how to provide them with the “right approach at the right time in the right place”. Training will be offered to ensure we all embed restorative approaches in our practice and support positive change through a Make Every Contact Count approach.
