Job Description

The successful candidate will have the opportunity to expand their knowledge, skills and work with Consultant Surgeons with an interest in advanced laparoscopic procedures, Upper GI Surgery, Vascular and Colo-Rectal Surgery.

The post is not recognised for training and therefore does not have educational and training approval from the Postgraduate Dean or Royal College. In all other respects the postholders have identical terms and conditions of service to the departments other SHO's. Clinical experience and research opportunities will be available for individuals wishing to progress in their career.

You will have the same role and responsibilities as the hospital’s other ST’s and you will receive training according to your needs and study leave will be available.

All candidates are required to have excellent communication skills.

The post follows a EWTD compliant full shift pattern.

Duties

The successful applicant will be placed in any of the following Surgical specialities and may have the opportunity to switch at 6 months into another Surgical speciality;

General surgery (Upper Gi)
Vascular
Colorectal

Duties will include:
Daily assessment / ward round of in patients

Supervision of junior members of the team, with regards to in patient management

Attendance at outpatients clinics and operating lists

Attendance on other departmental activities e.g. MDT surgical teaching

Audit M& M meetings

The applicant will participate in the on call rota (see further details below)

The Clinical Fellow will be expected to cover the Vascular and General surgical unit at middle grade (registrar) level. On call he/she will be supported by an FY1 and either an FY2 or a CT trainee and will answer to the on-call consultant. On call they will be required to cover the specialities of General Surgery, Vascular and Urology and in turn will be covered by the respective consultants in those specialities. The post-holder must respond promptly to calls from A&E and the wards (medical and surgical) to give advice and/or see and manage patients.

They would be expected to see urgent and emergency referrals from other disciplines/wards within the hospital. He/she must confer with the consultant on call and keep a record of such patients in the admission diary so that the patients could be appropriately followed up.

The Senior Clinical Fellow will keep an accurate and meticulous record in patient’s case notes and will supervise junior medical staff to do so. All entries in the case notes should be signed (with time, date and contact number). In addition he/she will provide clinical supervision and support to the junior medical staff.

They must keep accurate record of emergency admissions and referrals and formally handover to the on call team at the end of their shift. Handover must be formal, should follow the SBAR pattern (Situation, Background, Assessment and Recommendation) Similarly they must make sure that they have understood the patient’s problems and needs when receiving handover from somebody else.

They will supervise juniors in writing comprehensive but succinct discharge letters and summaries. They must ensure that VTE prophylaxis assessment is undertaken and that prophylaxis is given.

They must supervise, guide and teach junior members of the team in such skills as time management and team working.

The Senior Clinical fellow will be expected to participate in departmental academic activities and audit of mortality and morbidity.

They must demonstrate a willingness to learn and avail all opportunities to advance their clinical and operative skills. They will be expected to attend, assist and perform operations and procedures within their ability.

The Senior Clinical Fellow will undertake duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant(s), in consultation where possible with colleagues both senior and junior. Additional commitments arising under this paragraph are exceptional. In particular, the doctor will not be required to undertake work of this kind for prolonged periods or on a regular basis.

They must read and follow any protocols of medical cover laid down (either in writing or verbally) for the firm in which they are working.

In case of any difficulty ensure that advice is sought from a senior colleague.

NOTE: Specific duties may vary from time to time with the circumstances existing and the developing competence and interests of the members of the firm.

The post-holder will be expected to cover for absent colleagues in an emergency situation (e.g. sickness, injury, instances where an arranged locum does not arrive at short notice and similar situations).

1. a) General Duties:
2. i) Patients to be seen and assessed on the ward prior to surgery or admission to Intensive Care.
3. ii) Keeping such records as may be required. These must be dated and initialled, together with any other related administrative duties, including discharge and Clinic letters.

iii) Duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant(s) in consultation where possible with colleagues both senior and junior. Additional commitments arising under this paragraph are exceptional. In particular, the doctor will not be required to undertake work of this kind for prolonged periods or on a regular basis.

(iv) To read and follow any protocols and clinical guidelines laid down (either in writing or verbally) for the anaesthetic department.

(v) In case of any difficulties, either clinical or administrative, to ensure advice is sought from a senior colleague. (vi) To accurately gather data required for clinical audit and to participate in audit meetings. Keep a log book (electronic or written) of the cases anaesthetised. NOTE: Specific duties may vary from time to time with the circumstances existing and the developing competence and interests of the members of the Department.

Vascular team

The Bedfordshire and Milton Keynes Vascular Unit provides a supra district vascular service to Bedford Hospital, Milton Keynes and Luton & Dunstable Hospital catchment areas and is a designated arterial intervention Centre. The department provides the full spectrum of elective and emergency surgery, including endovascular aneurysm repair, urgent carotid endarterectomy, distal bypass for peripheral vascular disease, combined open/endovascular procedures and endovenous treatment for venous disease. The Vascular Surgeons run a 1:6 on-call Rota to which two surgeons based in Milton Keynes contribute. The Vascular team provide excellent surgical training, both in Vascular surgery and also some general surgery particularly hernias.

Colorectal team

Bedford Hospital is part of the West Anglia Cancer Network. We operate on in excess of 100 cancer cases last year at Bedford. There are five existing colorectal surgeons providing training in open and laparoscopic resection. There is a fully integrated laparoscopic (OR1) theatre, where all the colorectal surgeons carry out their theatre lists

In addition a full range of lower GI surgical services is offered: surgical management of inflammatory bowel disease and diverticular disease, abdominal and perineal surgery for anorectal prolapse and a full range of benign anorectal surgery including the management of complex fistula

A full range of endoscopy services is provided with upper and lower diagnostic endoscopy, stenting of oesophageal and colorectal lesions and therapeutic ERCP. We are a BCSP and a JAG accredited Centre.The department is committed to training surgical trainees in endoscopy

There is a purpose built colorectal outpatients department with modern consulting facilities

The Breast team

The Breast Unit provides training in a wide range of both Breast, Endocrine and Reconstructive procedures including: localisation for screening cases, wide excision and mastectomy, sentinel lymph node biopsy and axillary clearance. In addition there is exposure on the combined firm to General Surgical procedures including: open hernia repair and laparoscopic cholecystectomy. Tailored to trainee interest and experience, there are opportunities to develop laparoscopic hernia surgery and endocrine surgery (thyroid/ parathyroid). The clinics provide good, broad experience in general surgery and endocrine cases along with diagnostic one stop breast clinics, and breast cancer results clinics. We work closely with the Department of Plastic Surgery, providing the full range of breast reconstruction techniques.
