Southern Health NHS Foundation Trust plays a key role in supporting the discharge and ongoing care of patients from our local acute hospital. In order to improve the patient experience we have been asked to provide support to a programme of working relating to Medically Fit for Discharge patients in the South East area of Hampshire areas.

Many HCSW's have gone on to Band 4 Associate Practitioner roles or undertaken Nurse training to become qualified. In this role you will be supported in your Care certificate and to attend training courses as part of your ongoing professional development.
The multidisciplinary Enhanced Recovery and Support at Home team based at Aerodrome House are looking to recruit HCSW's to become integral part of our rehabilitation and re-enablement service covering a wide area. You will be part of a busy team providing varied day to day, patient focused short term packages of care working eight hour shifts over a seven days between the hours of 8am and 9 pm. You will support our extensive team of OT's, physiotherapists and nurses in providing community based support usually, although not exclusively, to the over 65 age group within their own homes.
Main duties, tasks & skills required
To deliver person centred care according to a patient's individual care plan under the supervision of qualified staff, adhering to Southern Health NHS foundation Trust policies, procedures and guidelines
