Job Description

We are excited to announce a new role based in the London Borough of Merton with a focus on supporting young people to transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Services. The post holder will also support families and work alongside teams and liaise with wider organisations ensuring wider system collaboration to wrap around support for our young people.

These new innovative roles have been developed to ensure our young people moving from CAMHS services into Adult services are sufficiently prepared and supported to navigate adult provision and networks, whilst maintaining their progress and promoting independence.

The aim of the CAMHS Transition posts is to support the successful transition of young people aged 17-25 into appropriate mental health services and to enhance the social and mental health outcomes of young people experiencing difficulties as they grow into adulthood.

It is acknowledged that the transition from child to adult services creates risks for young people who may fall through the systems due to not having the right support. This can result in poor mental health and social outcomes, leading to loss of human potential and increased burden on health and social care services over a lifespan.

This post will directly inform and update Transition Protocol between CAMHS and AMHS based upon the latest evidence of best practice. The post holder will also be responsible for supporting teams and provide training, where necessary, to educate and skill up professionals and stakeholders in the needs of those young people moving from CAMHS into adult services. Core to the role with be ensuring excellent collaboration between the young person and families to support the young person’s journey across services.

We are looking for an individual who has a strong track record in system wide working and communication across teams, who demonstrates excellent leadership qualities and someone who is inspirational and engaging with young people, staff and external partners.

The Transition worker posts are positioned within Tier 3 CAMHS teams. Tier 3 CAMHS is an CYP-IAPT service which provides targeted/goal focussed, evidence-based interventions to children, young people and their families up to the age of 18. CAMHS receives referrals for children and young people who have moderate to severe/complex mental health needs. Often a lower level/Tier 2 interventions may have already been utilised. Referrals are assessed initially by the Single Point of Access or may have come from the CAMHS Emergency Care Service (CECS). It is a multi-disciplinary team and clinicians are keen to ensure good outcomes for the young people within the borough.
