An exciting opportunity has arisen for a rehabilitation assistant to join our Community Stroke Rehabilitation Team (CSRT). The rehabilitation assistants support the multi-disciplinary team to provide patient centered support and rehabilitation to people who have had a stroke, mostly in their own homes.
The rehabilitation assistants are trained and supported by the therapists to deliver exercises programs, to assist people with functional tasks including personal care, cooking, shopping, exploring appropriate social opportunities for the people, supporting with communication difficulties and assessing weight.
Our rehabilitation assistants have a direct role in working with individuals to make a lasting and positive difference to a person's rehabilitation and life after Stroke. The team aims to support the early discharge from hospital to enable patients to receive the therapy they need at home for up to 12 weeks after discharge.
