We are looking to appoint enthusiastic and passionate nurses looking to be part of the process of improving the patient’s elective surgery journey. Both full and part time vacancies exist. A surgical or theatre background is preferred but not essential. The ability to work autonomously with a flexible attitude towards your work would be a sound foundation. Our service is nurse-led and dynamic. It is responsible for providing exemplary care for patients undergoing a large variety of surgeries. Our priority is to provide a high quality, focused and safe assessment service. This post would ideally suit a nurse keen to develop their professional profile and there are opportunities to participate in service improvement projects; you will be encouraged to play an active role in the teams' dynamic.

Superb communication skills, both written and verbal, are absolutely essential for this role. As skilled professionals you will be comfortable working as part of a wider MDT team. IT skills are essential. You will receive a supernumerary period and be supported by in-house training and competencies.

Ensure a comprehensive individualised Preoperative Assessment of the surgical patient.

Deliver patient-centred nursing care in a clinic environment aiming to achieve the highest possible standards of care delivery, in accordance with the Trust’s Strategy for Nursing and Midwifery; demonstrating commitment to Trust values and beliefs.

Deputise in the absence of more senior nursing staff and undertake clinic management duties as required

Co-ordinate the work of the multi-disciplinary team as required to provide care of individual patients

Promote best clinical and professional practice

Ensure efficient use of resources

Assist in the promotion of a conducive learning environment for all staff and students

To support more senior colleagues in the smooth running of the department.

Clinical
1. To conduct a holistic nursing assessment of the surgical patient to assist in the determination of their suitability for their surgery or procedure.
2. Order appropriate clinical investigations according to agreed protocols and act on any abnormal results.
3. Responsible for the assessment, planning and implementation of care required and assists in the interpretation of data relevant to the patient’s condition.
4. Assist with identification of patient’s complexities and needs with the patient and their carers, senior nursing staff and other staff as appropriate.
5. To ensure patients are suitable for their procedure and anaesthetic by carrying out a full assessment prior to the procedure.
6. To care for patients monitoring their clinical observations for any adverse events and reporting any deviances to a more senior colleague if necessary.
7. Identifies the priorities of care for patients.
8. Identifies realistic goals for patients.
9. Gives skilled care to patients and provides support to their carers.
10. Changes pathways and goals according to evaluation and discussion with the multidisciplinary team.
11. Demonstrates an understanding and skill in the care and management of the surgical patient.
12. Works closely with all members of the care team to ensure the needs of the surgical patient are met.
13. Contribute to the discharge plan to ensure a smooth and effective discharge for patients.
14. Undertake a nursing assessment of past medical, surgical history and ordering of appropriate clinical investigations according to agreed protocols.
15. Ensure patient fully understands the proposed procedure and further explain the information provided by the consultant or a member of their team.
16. Provide the opportunity for explanation, discussion, minimise fears and anxiety the patient may experience.
17. Ensure any contraindications to surgery are discussed with the surgeon, anaesthetist and patient.
18. Provide information about the pre-operative process and any specific pre-operative instructions, e.g. who to contact if patients want to cancel or are unwell and any fasting instructions.
19. Identify any special requirements for the surgical procedure.
20. Identify any cultural requirements and any communication or other special needs.
21. Assess the home support available to the patient post-discharge and identify any special requirements to facilitate prompt discharge.
22. Enable individuals to assist in the planning of their hospital stay through to discharge.
23. Document all assessment findings clearly and concisely.
24. Dispense drugs and treatments as prescribed according to the NMC guidelines and Trust guidelines for the safe handling of medicines.
25. Maintain and be able to supply evidence of continuing education as recommended by the NMC.
26. Acquire and maintain an up to date working knowledge of the all Trust Policies and Procedures.
27. Give advice on medication: what to continue and what to stop prior to admission, according to guidelines.

Operational/Management
1. Ensure deployment of the unit’s workforce on day-to-day basis meets the requirements of the service.
2. Organise time, equipment and staff to deliver pre-operative assessment care.
3. Delegate and assign activities and tasks appropriately.
4. Act as an advocate for the patient/client ensuring the provision of appropriate information and support services.
5. Effectively liaise with the multidisciplinary team and other departments to allow timely management of patient throughput.
6. Ensure the health, safety and welfare of the clinic staff, taking action where necessary and report issues of concern to the clinic sister or manager.
7. Identifies incidences of concern to either patients or staff and deals effectively with the consequences, completing appropriate documentation (for example DATIX).
8. Orientate new staff to the clinic and contribute to the clinic or department orientation programme.
9. Assist in the investigation of complaints as required.
10. Represent team at meetings and feedback information appropriately.
11. Support the education and development of all staff including students. Take part in education and training programmes as feed back knowledge gained.
12. Liaises with senior / managerial staff to bring to their attention any problems identified.
13. Promotes the principles of improving working lives amongst team, ensuring fairness and equity.

Communication
1. Communicates effectively with patients, their carers and co-workers.
2. Develop expertise in one clinical area as Link Nurse. Communicate up to date information or changes in evidence base for practice to the rest of the nursing team.
3. Demonstrates good practice in the handling of bad news to patients, carers and junior staff according to the Trust’s Guidelines for the Handling of Bad News.
4. Actively involve patients relatives and carers in their treatment and enable individuals to assist in the planning of their hospital stay through to discharge

Audit/Research

Educational
1. Participate in the development of an acceptable learning environment within the clinic.
2. Demonstrate a positive attitude to the mentoring of learners and junior staff.
3. Participate in the educational audit of the clinic/department.
4. To be a resource for students and junior colleagues, facilitating the development of junior staff.
5. Support and contribute to clinic-based teaching programmes.
6. Educate patients on health promotion.

Professional
1. Demonstrate a professional approach to work in accordance with the NMC Code at all times.
2. To have knowledge of and adhere to clinic, Trust policy and professional standards.
3. To maintain confidentiality, privacy and dignity surrounding the patient/client’s clinic appointment and treatment at all times.
4. Ensure that documentation is complete and up to date, in line with NMC and Trust guidelines.
5. To meet NMC revalidation requirements.
6. To identify own learning needs and ensure own professional development is maintained.
7. Keeping up to date with practice developments.
8. Ensure that mandatory and statutory training needs are met.
