Job Title: RGN – SMI Physical Health Checks Service

Hours of work: Fulltime (37.5 hours per week Monday to Friday) / open to part time hours if preferred

Remuneration: £38,000-£42,000 (per annum pro rota)

Contract length: Permanent

Responsible to: Integrated Services Manager

Accountable to: Head of Integrated Services

Location: Primarily working within General Practice within Haringey with some outbound visits to Care Homes, Supported Accommodation and
Home Visits

Job Summary

As a member of Haringey GP Federation Primary Care Team, the post holder will provide high quality clinical Interventions, Medicine Management, Severe Mental Illness (SMI)
Health Checks and support Wider System Pathways.

The post holder will facilitate in the delivery Haringey Serious Mental Illness (SMI)
Physical Health Check Service, working collaboratively with other agencies to ensure patients receive high quality and standards of care appropriate to their identified mental health needs. This role will focus on the initiation of the new SMI Physical Health Check
Service across Haringey. You will be working with several providers across several sectors: primary care, community health, hospital, social care and the voluntary sector.

Haringey GP Federation have been working with the North Central London Integrated
Care Board (NCLICB) to develop a pathway of care for people living with Serious Mental
Illness (SMI). The aim of this post is to increase the number of physical health checks performed with people with a SMI who may be at increased risk of COVID-19 and its subsequent complications, Improve the general number of physical health checks preformed in people with SMI in Haringey from 21% in 2018 to the national target of 60% and Holistic patient outcomes through collaborative working with the Barnet Enfield and
Haringey Mental Health Trust and Haringey Wellbeing Network.

The service aims to deliver on key national and local strategies to reduce unwarranted variation in the physical health outcomes of patients with serious mental illness (SMI) by:

PRIMARY AREAS OF RESPONSIBILITY

The post holder will work the Service Manager and Senior Integrated Services Manager to develop and operationalise the Physical Checks for Serious Mental Illness (SMI) pathway.
The post holder will also work closely with Barnet Enfield and Haringey Mental Health
Trust and the Haringey Wellbeing Network in providing physical and mental health care and help develop an integrated model of care.

(SMI ) care at all levels. To play a key role in developing and supporting advancing skills and competencies for nurses in specialist practice.

The post holder will have access to confidential information relating to patients, their carers, practice staff and other healthcare workers. They may also have access to information relating to the Federation as a business organisation.
