We are looking to recruit a dynamic, skilled and motivated Pharmacy Technician to support the PCN in Wandsworth, London. We have a team of established and experienced pharmacists and are now looking for technicians to enhance the service provided. You will be joining a highly functioning team with opportunities to implement your skills and knowledge and develop them further. This is an opportunity to work alongside pharmacists to ensure safe and robust medicines management processes for patients within the PCN and support to the implementation of policies and SOPs within practices across the PCN where appropriate.
Must be registered as a Pharmacist with the GPhC.

Job Description
Medicines Optimisation and Safety:
- Work alongside PCN Pharmacists to ensure safe and robust medicines management processes for patients within the PCN.
- Support with repeat prescribing to ensure the PCN is prescribing safely and appropriately according to local and national guidelines.
- Support with the monthly prescription requests for the care homes within the PCN.
- Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation.
- Utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
- Ensure patients have adequate monitoring and medication reviews.
- Work alongside the patient services advisors to handle medication queries from patients with support from PCN clinical pharmacists and other PCN colleagues.
- Work alongside the patient services advisors to handle medication related queries from external sources such as community pharmacists, district nurses, hospitals etc.
- Support PCN clinical pharmacist with structured medication reviews.
- Improve the safety and efficacy monitoring for patients prescribed medication.
- Promote lifestyle measures and interventions for patients at risk.
- Promote the uptake of appropriate electronic repeat dispensing and online ordering.
- Support the practice to respond to medication related alerts and audits.
- Support with medication reconciliation from inpatient hospital discharges, outpatient clinic letters and other transfer of care information and communication.
- Support with shared care prescribing, ensuring specifications of the agreements are in place prior to accepting and that patients are routinely monitored as per the agreements.
- Support with standardisation of the implementation of local and national policies encouraging self-care and reduction of the prescribing of over-the-counter products.
- Support with the delivery of the CCG Prescribing Incentive Schemes.
- Support with the delivery of the PCN DES / IIF where clinically appropriate.
- Support with delivery of vaccinations where clinically appropriate.
- Supporting antimicrobial stewardship within the PCN.
Management and Administration:
- Develop medication related policies and SOPs.
- Support to implement policies and SOPs at practices within the PCN where appropriate.
- Deliver supervision and training for pharmacy technicians as the team grows.
- Actively participate as a representative of the PCN in forums and meetings within the PCN and externally with stakeholders such as CCG where appropriate.
- Lead and/or support with quality improvement initiatives.
- Act as a role model and promote the PCN Pharmacy Service within the PCN where appropriate.
- Deputise as medicines management lead in absence of clinical pharmacist in practice.
Education and Training:
- Deliver structured or ad-hoc training to patient services advisors and/or other clinical and non-clinical staff on medicines management. This is to aid in the upskilling of staff within the PCN to allow increased efficiency of activity by members of the PCN.
- Enrol in, undertaking or qualified from an approved PCN Pharmacy Technician training pathway. For example the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)
- Supervise junior pharmacy technicians where appropriate.
