Job Description
You will be responsible for direct care delivery for people with a diagnosis of organic mental health illness and associated problems related to the Aging process, in particular, Dementia, Challenging Behaviors’ and Physical Disabilities.
The Walton unit provides an assessment and treatment service for older adults with organic illness. There is a multi professional team working at supporting the needs of the patients with the aim of fostering hope and quality of life,
You will carry out all relevant forms of care in support of and supervised/directed by a Registered Nurse and will support the Registered Nurses in the implementation and delivery of high standards of patient care promoting patient wellbeing following a ‘Person-Centred Approach’
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

- To support registered nurses in the implementation of a high standard of patient care promoting patient wellbeing following a ‘Person-Centred Approach’.
- Follow nationally accepted guidelines as directed when dealing with aggressive/abusive situations and attend the Managing Violence and Aggression courses as mandated whilst supporting patients, staff members, carers and the public in stressful situations
- Use judgement to identify challenging situations and defuse and distract patients to promote the safety of patients, other staff members, carers or other visitors to the ward.
- Report any concerns or problems to supervising nurse, particularly relating to individual patient safety.
- Support patients care needs in generic health resources both in patient stays and out-patient appointments as directed
- The post holder will participate as a member of the nursing team in delivery and monitoring of individual care/treatment plans and report changes to the registered nurse as appropriate.

- Support formal and informal patients, following treatment plans, Section17 Leave arrangements and risk assessments across hospital and community health based settings.
- Assist patients in meeting social, leisure and spiritual needs escorting patients as directed by the registered nurse in the local community taking into account any emerging risk requirement remedial actions.
- Ensure that all patients are afforded due respect, dignity and privacy.
- Maintain observation requirements of individual patients as directed on an intermittent or continual basis in line with NICE guidance.
- Display high levels of effective inter-personal skills using both verbal and non-verbal skills to support patients in highly emotional states and patients who cannot communicate due to poor cognitive condition / memory loss.
- Record care interventions, EWS, results in core records and report findings to registered staff.
