JOB SUMMARY
An exciting opportunity has arisen to join Shoreditch Park & City PCN as a Trainee Nursing Associate. If you are a dynamic individual, interested in developing your career in primary care then we would love to hear from you.
Groups of GP Practices in City and Hackney have recently come together to form 8 Primary Care Networks (PCNS) each covering a total population of between 30,000 - 56,000. The purpose of these PCNs is to enable GP practices to work together in a collaborative way to develop and deliver network-based services that respond to the needs of the local population.
About Shoreditch Park & City PCN:
Shoreditch Park & City PCN is a friendly group of five Practices supporting a diverse population in City & Hackney. The post holder will work collaboratively with a multidisciplinary health & social care team to support and provide holistic treatment and care for our diverse population groups. Tackling health inequalities are core focuses for the PCN, and we are looking for a candidate that will be able to work with supporting the needs of different patient groups – such as patients with learning disabilities, autism, special needs and those from diverse cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential. Specific training will be provided where required.
Our practices have established supportive and well-structured nursing teams in which the successful candidate will sit within, while being able to work independently and use their own initiative.
This is a developmental role over 2 years. You will work towards successful completion of a Level 5 Foundation Degree equivalent qualification as a Nursing Associate and registration by the Nursing & Midwifery Council (NMC). You will develop and maintain knowledge, skills and competencies related to the role of a Nursing Associate through your apprenticeship within the PCN as well as through completion of the Nursing Associate Apprenticeship training program. This includes working in health care settings and delivery of direct person-centred care.

The Nursing Associate Apprenticeship Programme combines and integrates academic and work-based learning through close collaboration between employers and education providers. As a Nursing Associate Apprentice you will be based as an employee within Shoreditch Park & City PCN, and also undertake placements in a variety of clinical settings. This will enable you to gain a wide appreciation of many health and care contexts, which will enable you to fulfil all the requirements of the Foundation Degree Apprenticeship Nursing Associate Program.
On successful completion of the Programme you will be equipped with the appropriate knowledge, skills, attitudes and behaviours to ensure competence at the point of qualification. Key areas of practice for Nursing Associate apprentice students: The training programme emphasises the role that apprentice nursing associates can play in life-course (pre-conception to end of life) approaches towards health and well-being and the ways in which they actively contribute to the delivery of holistic care. Holistic care, across life-course, is a whole-person approach which considers, and equally values[1], physical, psychological and public health needs, learning disabilities, social, economic, spiritual and other factors in the assessing, planning and delivery of care.
The overall outcome from the training programme is a Nursing Associate that is fit to practice in the widest range of settings as well as being equipped with the specific knowledge, skills and capabilities required for the context in which they have trained and are employed.
Key Responsibilities relating to your learning.
- To attend University and clinical placements as set out in the foundation degree program
- To complete and pass academic modules and clinical placements to the required standard in order to progress through the programme
- To ensure timely achievement in accordance with the academic programme requirements
- To participate in tripartite reviews, between the University and the employer. These are a requirement of the apprenticeship.
- To complete and pass the End Point Assessment.
- It is expected that the level of responsibility will increase throughout the employee’s time on the foundation degree program, with directions from their work based supervisor.
- To gain knowledge, skills and experience during the period of the apprenticeship in a wide range of clinical areas.
- Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the on-going assessment, planning, management and evaluation of care to include:-
Main Duties and Responsibilities In Line with learning and competency.
Patient and Clinical Care.
- Be actively involved in the provision of health education and self-management advice to patients, relatives and carers.
- Supporting individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence through awareness raising and care navigation to other professionals as appropriate.
- Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures.
- Assist in the implementation of appropriate interventions to meet the specific physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers.
- Assist in the assessment of and contribute to the management of risk within the environment where care is being administered.
- Carry out ‘NHS Health Checks’ and clinical observations including, blood pressure, temperature, respirations, pulse and cholesterol testing. Measure and record patients’ height, weight, and BMI.
- Carry out urinalysis of urine samples for in house testing in accordance with practice protocols.
- Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
- Give lifestyle advice to patients (including diet, weight and exercise) making any necessary referrals to local support services as required.
- Give smoking cessation advice and initiate smoking cessation therapy.
- Assist in providing minor-surgery sessions, coil fits and implant fittings.
- Undertake appropriate investigation, clinical observations and interventions following agreed guidelines for patients with chronic diseases, CHD, Diabetes and Hypertension.
- Carry out cardiology diagnostic testing for internal and external patients.
- Perform spirometry tests for patients in line with practice protocols, ensuring that a PSD is used for the Salbutamol nebuliser.
- Meet the needs of patients presenting for non-complex wound care and use dressing formulary. Carry out stitch/ suture removal in accordance with practice protocol.
- Take venous blood samples from patients in line with practice protocols and local hospital pathology protocols.
- Promote health and prevent ill health by understanding the evidence base for immunisation, vaccination and herd immunity.
- Document and administer influenza and pneumococcal vaccinations according to a PSD.
- Document and administer B12 injections according to a PSD.
- Undertake clinical tasks including cannulation, venepuncture, ECGs.
- Accurately record nutritional and fluid intake.
- Ensure the privacy, dignity and safety of individuals is maintained at all times.
- Develop understanding of caring for individuals with particular conditions for example dementia, mental illness, learning disabilities.
- Recognise patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health.
- Demonstrate good understanding of the Mental Capacity Act and Deprivation of Liberties, applying principles to everyday practice or seeking advice from the registered care professional as required.
- Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals.
Performance & Quality.
- Develop skills in relation to coaching/teaching, individuals/carers/other staff.
- Assist with the implementation and monitoring of clinical standards and outcomes.
- Demonstrate the ability to recognise changing priorities seeking advice and guidance from the registered nurse or other registered care professionals as appropriate
- Develop a working knowledge of other providers’ resources and referral systems to ensure individual’s needs are met, within parameters of practice.
- To be able to cope and work in stressful atmospheres at times including managing time and seek support where necessary from your clinical supervisor and /or the University.
- Engage in reflective practice including management of self and reflection on own reactions, asking questions and reflecting on answers given.
Administration
- To maintain accurate records of all patient consultations and related work carried out at each clinical session, including computerised data collection.
- To contribute to the development of service standards.
Financial and Physical Resources
- Make best use of time and resources in order to maximise the benefit to both the patient and organisation. In this way the post holder will support the organisation in delivering value for money.
- To be responsible for monitoring stock levels in own service area and request new equipment as appropriate.
Knowledge and Skills
Supervision
- To take part in student placement schemes offering demonstrational opportunities for both Nursing Associate students and students from other disciplines as appropriate.
- To take part in induction programs for new staff.
- Actively contribute own development and learning through supervision and reflection.
Communication and Relationships
- To work effectively with other staff and members of the community (e.g. District Nurses, care staff, carers and relatives) in order to implement patient care plans. To ensure good communications are maintained at all times to achieve the desired outcomes.
- Report back and share information with the registered nurse on the condition, behaviour, activity and responses of individuals.
- To liaise with administration staff around appointments and communication with patients and professionals.
- To establish and maintain positive interpersonal relationships with other staff which are characterised by open communication, trust and respect.
- To ensure effective communication of sensitive and confidential information to patients, relatives and carer’s where there maybe barrier’s to understanding, which requires interpersonal and negotiating skills including participation at Multidisciplinary Team meetings (MDT).
- To demonstrate empathy with clients, carers, families and colleagues ensuring that effective communication is achieved particularly when barriers to understanding exist.
- To deal with patient expectations and needs sensitively and persuasively.

Desirable requirements
- Qualifications and Credit Framework Level 3 in care or equivalent in terms of both academic attainment and previous care experience
- Essential induction training and experience – care certificate or equivalent
- Previous experience of working within a care setting
- Understanding of evidence based practice
- Understanding of basic physiology, e.g. normal vital signs, fluid balance nutritional requirements etc.
- Evidence of involvement in the support and development of less experienced staff
- Evidence of recent work-based learning or self-directed learning.
