Job Description
The post holder will have successfully completed an NVQ in Health and Social Care or equivalent and will work under the supervision of the Bridging Service Team Lead as part of the Waltham Forest Integrated Discharge Hub.
The post holder will work in partnership with NELFT community services, Waltham Forest Social Care and other agencies to provide short term, responsive and quality homebased care and rehabilitation for service users.
The post holder will be responsible for supporting the day to day running of The Bridging Service to deliver care in line with agreed care plans and coordinate staff.
The service offers short-term homebased care with the aim to:
a) Facilitate timely safe discharge from acute hospital care
b) Help prevent unnecessary admissions to care homes
c) Help prevent unnecessary hospital admissions
d) Help to regain confidence with living skills or support with long term care needs until such time a Social Services / private package of care can commence.
The post holder will supervise Support Workers and support the Bridging Service Lead, being an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.
Using a reablement approach the post holder will facilitate service user independence and empowerment as far as possible with regard to existing medical conditions and disabilities and assist the service user to learn or regain skills with personal and domestic tasks as part of their therapeutic plan.
- To give holistic care to service users in their own home.
- To plan and organise Bridging Service daily rosters, coordinating the shift to ensure all visits are completed and staffs safety is maintained.
- To brief Team Lead on all relevant developments for each service user.
- To participate in a duty rota to provide care for the service users to include unsociable hours.
- To assist in the administration of medication, if necessary, with suitable training.
- To have knowledge relating to Trust protocols and procedures and adhere to them, particularly manual handling.
- To be responsible for providing accurate records of information required by the Trust for audit purposes.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement – London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,108 and a maximum payment of £5,177 per annum pro rata).
- To maintain accurate records in the service users own home or elsewhere. Identify and
report any changes in service user care needs or changes in physical or emotional well-being to the Team Lead.
Provide information to assist local resolution and complaints when required.
- To be skilled in communication, establishing and maintaining good relationship with
service users, carers, members of the wider health and social care team or other agencies.
- To maintain, update and develop personal, professional knowledge and skills achieving
high standards of service users care at all times. Reflect on practice regularly and plan personal development in order to achieve growth development. Access supervision from the Team Lead and other staff to enhance own practice.
