Job Description

Adferiad Recovery delivers a new, flexible, and co-ordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance misuse conditions and related issues. We harness the talents and experience of our founding and long-established leading Welsh Charities CAIS, Hafal, and WCADA in their fields, enabling our projects to meet the needs of our most vulnerable with a single, unified, and comprehensive approach.

An opportunity has arisen for a passionate and self-motivated individual to join the team as a Support Worker based Pembrokeshire

Hours: Part Time (8 hours minimum) up to full time 37.5 hours per week

Salary: £10.25 p/h (08.00 – 20.00 Monday-Friday)

£12.90 p/h enhanced (20.00 – 08.00 Monday -Sunday)

£11.08 p/h weekends (08.00 – 20.00 Saturday & Sunday)

Expenses: Training, DBS check (Enhanced Children & Adults) and SCW Registration fee

Holidays: 25 days per year plus 8 public holidays (pro rata)

Period of Notice: 1 week during probationary period; 4 weeks thereafter

Probationary Period: 6 Months

Based at: Various communities across Pembrokeshire – Haverfordwest, Tenby, St. Dogmaels, Newport (Pembs), Fishguard, Milford Haven, Pembroke Dock, Saundersfoot, Narberth.

Responsible to: Pembrokeshire Service Manager (Adults)

Job Purpose:

Under the direction of the All-Wales Domiciliary Care Registered Manager Adferiad operates domiciliary care services including domiciliary home care services, palliative care services and respite services for Carers. In line with our commitment to principles of empowerment, self-management, wellbeing and recovery and ensuring compliance with internal and external quality standards inspection and regulation (including CIW).

About this job:

This service is delivered in people’s own homes within the local communities of Pembrokeshire. This service is generic and therefore supports individuals with a wide age range and variety of support needs - including individuals with physical disability, mental health, learning disability, frailty and their Carers.

As a member of our dedicated team, working as a Support Worker you will be working 1:1 (occasionally 2:1) as part of a dedicated team to provide support with day-to-day living including support with personal care, medication, manual handling and accessing the local community to promote social inclusion, while focusing on promoting the wellbeing, rights and independence of the individual.

Rosters are created and managed to ensure continuity of support staff for individuals while minimising travel and promote good work/life balance for support staff.

Role Responsibilities:

All Adferiad Support Workers will have and be given the qualifications, experience and competencies commensurate with the levels of care they are expected to provide and in accordance with the Social Services Wellbeing Act:

This includes supporting individuals if required with:

· All aspects of personal care

· Mobility and transfers – all staff are trained in accordance with the All-Wales Manual Handling Passport.

· Administration of prescribed oral medication - all staff must complete Agored Cymru accredited Medication Training

· Social Inclusion - accessing the local community and participating activities of their choice.

And:

Completion of detailed observations and reports in relation to individuals which can be shared with health professionals to ensure adequate care arrangements are in place from statutory/voluntary services to meet the individuals needs if continued care and ongoing support is required.

· Maintaining records and statistics required to monitor and evaluate the effectiveness of the service

· Communicate effectively with Health Care Professionals, families, and Carers

· Continuous professional development, attending training courses and other as required.

· Carrying out all reasonable tasks commensurate with the post as directed by line manager

· Implementing the Adferiad Equal Opportunities Policy and all other Adferiad policies and procedures

· Working to a rolling shift pattern – including mornings, evenings, and weekends.
