The MDT Pathway Involvement Officers will support Northumberland Recovery College (NRC) and VCSE Mental Health Alliance to maintain and continue to meet the needs of Community Mental Health Transformation (CMHT) in the following ways:
- Engagement with service users

Engagement in local communities
- Establishing and developing links with Northumberland PCNs

Establishing and developing links with CNTW clinical teams
-
Attending MDT meetings to identify and recommend NRC activities to support individuals
-
Engaging and supporting pathways from CNTW and PCNs to NRC activities
-
To measure the impact/effectiveness of the work on individuals and CNTW Primary/Secondary Care services, through feedback surveys and/or focus groups

To be aware of the needs of individuals living with SMI in Northumberland, specific to their community.

To attend MDT meetings in both Primary and Secondary care.

Maintain knowledge and understanding to co-ordinate and manage the pathway of individuals to appropriately support their mental wellbeing in their own community.

To use knowledge gained from MDT meetings in Primary and Secondary care, and use high levels of communication to proactively support and encourage individuals to access relevant activities and courses.

To have the ability to make proactive recommendations for non-clinical support enhance the patients pathway.

To have an extensive knowledge of the most up to date NRC activities/course timetable and to use this knowledge to make appropriate recommendations to the patient pathway.

To communicate information to a wide range of health professionals and use own initiative to seek professional advice as needed.
- To gain knowledge of medical terminology as appropriate (for information purposes only).

To take time to understand individual cases and make recommendations for non-clinical community-based support.

To ensure all individuals are enrolled with NRC and provide support with this process where required.

To be aware of and adhere to appropriate risk escalation and safeguarding procedures.

If appropriate implement any ideas that support the improvement of the pathway with the support of the wider MDT team.

To have an understanding of any issues that may impact on the pathway.

To regularly meet and keep clear communication with NRC and VCSE MH Alliance colleagues and those in MDTs.
- To work with the MDT members to ensure that appropriate action follows the MDT meeting.

To have the ability to confidently speak up in an MDT meeting in situations where the other members are consultant level.

To keep up to date data on number of patients referred to NRC/VCSE MH Alliance.

To measure impact and evaluate patient experience using online survey (JISC) and focus groups with Primary/Secondary care.
