Job Description We are looking for two Integrated Neighbourhood Project Managers to support the development and delivery of Integrated Neighbourhoods across Cambridgeshire South. Your role will be to bring partners and communities together, helping Integrated Neighbourhoods to identify how they can improve care and support, and then to deliver this change and transformation. You will act as a relationship builder and connector helping to join partners up and try new ways of working at a time of significant pressures and challenges. You will provide transformation and project management expertise to make positive changes, build strong and sustainable foundations for Integrated Neighbourhoods and help tackle inequalities. You will work closely and collaboratively with the other Integrated Neighbourhood Project and Programme Managers and may line manage project support officers. Work closely to build relationships and bring partners and communities together to make positive change Work closely in partnership with clinical leaders, the acute sector, community services, health commissioners, councils, voluntary and community organisations, and the public to support the development of the service design Provide transformation support and project management for the Integrated Neighbourhoods programme of work Supporting the translation of a vision into a coherent operational model and project delivery Review population health data and support public engagement to determine how the local citizens want their care and support to improve Be responsible for supporting the service redesign and delivery of specific projects within the Integrated Neighbourhood Promote equality and reduce inequalities in your work Facilitate partnerships across all partners and supporting cross organisational and cross boundary working You may line manage project support officers and will work closely and collaboratively with the other Integrated Neighbourhood Project and Programme Managers.
