Job Description

Join the Revolution in Patient Care: Become a Remote Monitoring Clinical Support Team Member Today

The Clinical Support Team Member for Remote Monitoring will manage patient care virtually, offering support for those with both short and long-term health issues.

This role is crucial in helping to prevent declining health, reduced quality of life, and hospitalisation, which often leads to more extended stays.

The Team Member will monitor patients closely, responding to any signs of deterioration and escalating concerns to Remote Monitoring Clinicians for timely intervention and recovery in the community setting, avoiding the need for hospitalisation.

KEY RESPONSIBILITIES

The post holder will:

1. Have an appropriate experiential health care background, such as a health care assistant, therapy assistant, or emergency medical technician.

2. Work with Remote Monitoring Clinicians supporting high-level remote monitoring for patients with short and long-term conditions. Escalate to Remote Monitoring Clinicians when markers for deterioration are evident on submission of abnormal observations by patients via the Docobo system. Thereby ensuring patients on the caseload who are ‘alert’ have clinical contact urgently. This ensures that care is delivered in the community setting and aids in the prevention of unnecessary hospital admission.

3. Work with nursing and residential care partners to ensure optimum health outcomes for their residents.

4. Work on shared caseloads with Integrated Care System (ICS) by acting as an integral member of the community team with a shared vision for developing caseloads as required by Integrated Care System partners.

5. Possess experience working within the telemedicine field to support the delivery of clinical care remotely, working under the direction of the Remote Monitoring Team Leader and Remote Monitoring Clinical colleagues when required.

6. Deliver health promotion advice under supervision from clinical colleagues via the Remote Monitoring model, e.g. chronic disease management self-help signposting.

7. Work in alignment with Primary Care, for established practice populations, toward a shared vision of seamless management for patients with short and long-term conditions within the Primary Care and the Community setting.

8. Work with ICS partners in Primary and Social Care to further develop the Remote Monitoring model for cohorts of patients managed within local populations.

9. Maintain accountability for practice to the Remote Monitoring Team Leader following all EBPC Policies and Procedures.

10. Maintain legible, accurate and up-to-date records that are dated and signed in accordance with EBPC Policies for standards on record keeping.

11. Contribute to the ‘onboarding’ of patients with short and long-term conditions to the Docobo patient management platform.

12. Contribute to the ‘discharging’ of patients with short and long-term conditions from the Docobo patient management platform.

13. Work supervised with patients, their families and carers, managing referrals according to need and priority.

14. Escalate complex clinical data that has alerted on the Docobo patient management platform to Remote Monitoring Clinical colleagues.

15. Understand and support the achievement of EBPC’s business plan objectives and performance targets.

16. Identify the potential for service developments, risks and deficits and inform the Remote Monitoring Team Leader making recommendations based on vocational knowledge.

17. Be accountable for practice taking every reasonable opportunity to sustain and improve knowledge and competence.

18. Assist the Remote Monitoring Team Leader in managing the effective use of resources within the Remote Monitoring team and participating in regular review meetings with the Team Leader.

19. Have experience with the Docobo patient management platform or similar.

20. Have experience with other clinical patient management systems: Adastra, EMIS, and Connected Care.

PATIENTS CARE

The post holder will:

1. Use telemedicine knowledge and skills to remotely monitor the physical and psychosocial needs of a defined client group.

2. Use telemedicine and healthcare knowledge to respond to alerts raised by the Docobo patient management platform. Urgently escalating patient alerts to Remote Monitoring Clinicians to action.

3. Work with partners in nursing and residential care to improve the health outcomes of the residents and so prevent unnecessary hospital admissions or extended in-patient care episodes.

4. Assist Remote Monitoring Clinicians who are dealing with complex patient situations. Thereby aiding the establishment of a therapeutic relationship with individual patients using Remote Monitoring services.

5. Use telemedicine assessment tools/skills to ensure an appropriate level of therapeutic supervision so that patients with complex needs are referred/escalated via the appropriate care pathway.

6. Conduct patient encounters with sensitivity and act as a patient advocate, maintaining patient confidentiality.

7. Deliver patient-centred care, respecting diverse cultural backgrounds and requirements.

8. Work in partnership with the patients to empower them to make informed choices about their healthcare and support choices.

9. Offer a supportive service to patients and their carers using the Remote Monitoring service.

RESPONSIBILITIES FOR PEOPLE AND TRAINING

1. Be responsible for providing and maintaining a learning environment and maximising opportunities for education and development in the clinical area to enhance individual development and performance in delivering high standards of care.

2. Act as a Remote Monitoring resource and assist in mentoring new Clinical Support Team Members in the Remote Monitoring environment. Take part in the process of clinical supervision and participate in clinical supervision and mentorship of others.

3. Take responsibility for your personal development; maintain competence, knowledge, and skills commensurate with the role.

4. To be actively involved in clinical staff recruitment in the Remote Monitoring Team when required by EBPC.

OTHER FACTORS

1. Maintain personal knowledge, skills, and competency through appropriate CPD, training and development opportunities.

2. Keep up to date with new developments and legislation.

3. Using IM&T support systems.

4. It may be necessary to change the days worked as the service develops and grows; therefore, flexibility is essential.

CRIMINAL RECORDS BUREAU – DISCLOSURE
