Overview

We are looking for a dynamic, forward-thinking Consultant Physician to complete a team of nine (including new appointments) covering East Yorkshire for our NHS Client. The Directorate would equally welcome applications from existing and newly trained Consultants.
This post is based in the Elderly Care and Rehabilitation Department at the NHS facilities with opportunities to work across site.
The successful candidate will be encouraged to work as part of an established multidisciplinary team inputting and helping to develop new models of care within an acute frailty pathway. The East Riding of Yorkshire, or simply East Riding or East Yorkshire, is an area in Northern England. The name is traditionally and geographically a reference to the easternmost of the three subdivisions (called Thrydings or Ridings) of the traditional county of Yorkshire. The traditional cultural and historic geographic county, the East Riding of Yorkshire borders the North Sea to the east of the county. To the north across the River Derwent is the North Riding of Yorkshire, and to the west across the River Ouse is the West Riding of Yorkshire.
In terms of neighbouring local government administration, the East Riding of Yorkshire unitary authority adjoins North Yorkshire to the north, north-west and to the west, South Yorkshire metropolitan area to the south-west and Lincolnshire across the Humber Estuary to the south. The City of Kingston upon Hull is a separate unitary authority immediately to the south and forms part of the ceremonial county area. Other councils areas adjacent to the unitary authority include North East Lincolnshire, beyond the Humber Estuary; North Lincolnshire, beyond the Humber and on land; Hull, Doncaster, Selby, York, Ryedale and Scarborough.
The landscape consists of a crescent of low chalk hills, the Yorkshire Wolds, surrounded by the low-lying fertile plains of Holderness and the Vale of York. The Humber Estuary and North Sea mark its southern and eastern limits. Archaeological investigations have revealed artefacts and structures from all historical periods since the last ice age. There are few large settlements and no industrial centres. The area is administered from the ancient market and ecclesiastical town of Beverley.
Major sporting and entertainment venues are concentrated in Kingston upon Hull, while the seaside and market towns support semi-professional and amateur sports clubs and provide seasonal entertainment for visitors. Bishop Burton is the site of an agricultural college, and Hull provides the region's only university. On the southern border, close to Hull, the Humber Bridge spans the Humber Estuary to enable the A15 to link Hessle with Barton-upon-Humber in North Lincolnshire.
In 2019, there were 601k residents in East Riding of Yorkshire county with an average age of 42.2 years. Population density was 242 residents per square kilometer. Population grew by 5.8% since 2002 and population average age increased by 2.6 years in the same period. Based on Census, there were 2.31 residents per household in 2011.
Department
The elderly care and rehabilitation department at the hospital currently provides:
Acute medical care to frail older people in a 12 bedded Frailty Assessment Unit, a 24 bedded elderly care ward and a 12 bedded ward for elderly patients with dementia and/or delirium .
Acute and rehabilitation stroke care on a 30 bedded unit (HASU being implemented in early 2020)
Joint Orthogeriatric care on the hip fracture unit
Specialist clinics in movement disorders and stroke and rapid access ‘Frailty’ clinics offering Comprehensive Geriatric Assessment within a multi-disciplinary team approach in the Frailty Ambulatory Assessment area.
Supervision and training for medical students and junior medical staff
General Duties- guideline only
The post holder will contribute to the development of the Frailty Service establishing an effective multi-disciplinary team, to provide early assessment and management of the needs of frail older patients.
The post holder will build on the existing good relationships with primary and community care services so that frail, older people have risk factors identified and can be signposted to services that will support them to maintain their independence as close to home as possible and reduce the incidence of further crisis. This may include co-ordination of specialist multi-disciplinary meetings which include primary and community care services to ensure that patients have robust care plans which help them to stay healthy at home for as long as possible.
The successful candidate will be encouraged to develop any particular interest she or he has that complements the needs of the service and a timetable will be agreed in advance, to reflect the particular interests of the applicant, and the specialist interests of the existing Consultants, together with the needs of the Trust.
Job Plan
Regular job planning meetings will be undertaken with the post holder to ensure that the appointee is remunerated for any additional activities undertaken and that the job plan accurately reflects the overall workload undertaken by the post holder.
The supporting professional activities (SPAs) allocation within the provisional job plan is an illustrative guide and will be finalised following individual agreement with successful candidates. Further discussion on supporting professional activities will depend on the Trust's requirements and the individual's particular expertise.

On Call Commitments
On call commitment is currently 1 in 12-week days and 1 in 12 weekends. The average number of acute patients admitted on call is approximately 30 in 24 hours. Weekday on call is 17.30 – 22.00 resident on site and then available until 08.00 the following morning.
Weekend on call is either 09.00 – 12.00 noon or 13:30 – 22:00 resident on site and on call 22:00 until 09.00, Saturday and Sunday.
Junior doctor support during the day consists of:
- 1 x SPR
- 3 x CMT/F2
- 1 x F1
- 1 x SPR
- Plus 1 x F2/CMT at night
Specialist on call back up, available on site, is provided for critical care, cardiology, renal medicine and acute GI bleeding. Out of hours advice is available from hospitals within the network for infectious disease, haematology, oncology, acute neurology, cardiothoracic surgery and neurosurgery. Tele stroke is also available for stroke advice.
Salary
This is as described in the Medical and Dental Terms and Conditions, in line with the Consultant Contract (2003). The current full time salary scale (as of July 2020) ranges from £84,559 - £114,003 a year with eight thresholds.
The on call supplement is Category A and attracts a supplement of 5% of basic salary.
Office Accommodation
Office accommodation will be provided for the appointee with secretarial support.
Management Responsibility
All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the existing clinical team through, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
Teaching/Research
The consultant will be required to participate in programmes for teaching clinical students, training junior doctors and in clinical examinations. The post-holder’s contribution to teaching, training and research will be included in the regular job plan review. Teaching will take place during programmed activities and flexibly at other times.
- To provide conditions for improved training opportunities in line with national and local recommendations arising out of GMC and related requirements
- To supervise and manage the work of his/her junior medical staff and participate in the training and development of these staff to the standards required by the Royal Colleges.
Clinical Governance & Audit
The consultant will participate in clinical governance activities, including clinical audit, clinical effectiveness, risk management, quality improvement activities as required by the Trust, and external accrediting bodies. There are 6 half day clinical governance meetings per annum which substitute regular fixed sessions. There is a weekly consultant meeting before MDT on Friday to discuss any issues during the week and to plan for next week.
Personal & Professional Development
The post-holder will be required to keep himself/herself fully up-to-date with their relevant area of practice and be able to demonstrate this to the satisfaction of the Trust.
The Trust supports the requirements for continuing professional development as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities.
Medical revalidation is the process by which the General Medical Council (GMC) confirms the continuation of a doctor’s licence to practise in the UK, provides greater assurance to patients, the public, employers and other healthcare professionals that licensed doctors are up-to-date and fit to practise. It is a key component of a range of measures designed to improve the quality of care for patients.
The Trust supports Royal College guidance on provision of mentors for new consultants in line with GMC recommendations. All newly appointed consultant staff are provided with a list of locally trained mentors who are available for ongoing support as required.
