Boughton Surgery has an exciting opportunity for a Care Co-ordinator to join our team, working up to 37.5 hours per week with flexibility to cover holiday and sickness at short notice.
The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.
The Care Coordinator’s role will support the clinical directors in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centered.
Main Duties include:
- Responsible for Subject Access Requests to process in line with GDPR
- Process private medical report requests
- Apply rota for GPs, Nurse Practitioners and Nurses
- Maintain practice schedule of audit requirements
- Process patient documentation upon receipt at the surgery into patient records
- All general administrative clerical duties, including typing, logging staff annual leave, letters, (BMA, Firearms), photocopying, filing and outgoing post etc.
- Fire alarm and water testing
- Process and update registration links and workflow SCRs on a daily basis, liaising with PCSE to solve any queries, including deductions, FP69 notifications and GP2GP transfers
- Process patient records coming in or leaving the practice
- Updating patient data bases.
- Provide holiday / sickness cover as and when requested including attendance at practice meetings as minute taker.
- Carry out tasks required to support PCN
- Organising of patient care including data inputting for suspected cancer, mental health, 2 week referrals, patient safety incidents, newly diagnosed cancer, end of life care, supportive care, complex patients requiring caseload support
- Provide support to Management team with all aspects of administrative work
- Maintain confidentiality at all times
- Maintain awareness of and compliance with the Practice Procedural Guidelines and General Health & Safety Regulations within the Practice
- Undertake any further duties that may be required from time to time as requested by the Partners or Management team.
If you are a skilled Care Co-ordinator with excellent person-centered planning experience, with local knowledge of key support and advice and support to our patient population, then we'd love to hear from you.
You will need to have the following skills / qualifications :
- You will have ideally have experience of working within healthcare or the voluntary / community sector, supporting vulnerable groups;
- Applicants must have administration experience
- Applicants must be computer literate
We are a rural practice within the village of Boughton and cater for approx 3,500 patients with a team of 3 GP partners and 20 staff both clinical and administrative.
This is a new post within Fens & Brecks Primary Care Network that will be based at Boughton Surgery who are part of the above network of 6 GP practices.
