Permanent Contract | £32,909 to £37,231 per annum, pro rate (Scale I/J) | Attleborough Community Hub, Attleborough

Level 1: £32,909 to £34,723 per annum, pro rata (Scale I)

Level 2: £35,411 to £37,261 per annum, pro rata (Scale J)

We have three positions available within our team, please specify in your application which post(s) you are applying to:

Position 1 & 2: 37 hours per week

Position 3: 22.5 hours per week

We offer a £2000 welcome payment for qualified and experienced Social Workers (grade J and above), Practice Consultants and Team Managers new to Norfolk County Council.

For experienced Social Workers new to Norfolk County Council we promise to try and match or better your existing salary, providing its in the spectrum of the grade advertised.

It is an exciting time to join our team of dedicated and passionate social workers. We are looking for a Social Worker to join our Follow On team, this will be primarily working closely with patients who have been discharged from hospital by our ‘Home First Hub’ but require long term care planning.

Our team is experienced with working with complex cases, have a sound knowledge of the Care Act (2014), Mental Capacity Act (2005) and Safeguarding, as well as excellent time management and IT skills. Any new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures. Covering South Norfolk, our team of Social Workers, Assistant Practitioners and Occupational Therapists use the Care Act (2014) to assess and provide support to adults living in the community.

Who are we?

Integrated Community Teams Care incorporating Older People and People with Physical Disabilities

Our teams work with older people and adults with a physical disability. Our teams of social workers, assistant practitioners, Occupational Therapists and reablement staff from Norfolk First Response work across all five localities. In the east we work closely with colleagues from East Coast Community Health Care. Our senior management teams are made up of Operational Directors and Heads of Integrated Care who either work for Norfolk County Council or Norfolk Community Health & Care (NCHC). It’s a really integrated environment.

We work as part of the Integrated Care System and are also aligned to Primary Care Networks which means the integrated team structures benefits hugely from great collaboration with primary care colleagues. Our teams are strong voices for social care and social work within our integrated model and ethos of prevent, reduce and delay. The teams promote a supportive and learning culture with reflective practice and other learning opportunities such as practice teaching and social work apprenticeships encouraged.

We also have an Integrated Home First Hub of integrated health and social care staff which facilitates discharges from hospital. Our dedicated team facilitates discharges from Norfolk acute, mental health and community hospitals. We are also part of the Norfolk Emergency and Admission Avoidance Team (NEAT) which is an integrated team with health colleagues to prevent admissions to hospital.

Kate Pontin, Head of Integrated Care said: “We’re a friendly team with 3 office bases where we meet up when we’re not out and about. We have online chat groups, locality well-being meetings, reflection meetings, team meetings and provide good quality reflective supervisions. We also have a good induction programme, where any new staff are aligned to a supervisor on day one, and also have an aligned practitioner buddy for day-to-day support. We really encourage staff development and often take on students and apprentices. The new ideas they bring helps us to reflect on why we do what we do and provoke professional curiosity which helps us all grow and develop.
