The Condition Management Company is focused on delivering effective treatment to meet the individual needs of each client. Through our team of highly qualified and experienced clinicians, we work with each individual to design and deliver treatment packages focused on their needs and aims, putting the patient at the heart of service design.
We are looking for a skilled Psychological Wellbeing Practitioner to join us as a Wellbeing Navigator, working from our Nottingham location.
The Condition Management Company is led by clinicians with over 60 years in combined clinical experience and service design, across Nottingham and further afield. Our team have developed, operated and evaluated large scale services for the NHS and Department of Work and Pensions. We have also been involved in specialist service development, produced training programmes and provided performance consultation to other clinical services. Now we have taken the opportunity to focus on developing a local, personalised service with clients at the heart of the service.
The Wellbeing Navigator will primarily work on our maternity support pathway, understanding the impact of traumatic events on clients and designing a holistic client-centred package of care. You will remain an ongoing point of contact for clients throughout their involvement with the service. At other times, you may be working with IAPT clients utilising suitable low intensity interventions, or supporting our psychotherapists and clinical psychologists with our private clients.
We’re looking for a qualified PWP with a high degree of professionalism and sensitivity, who is interested in getting involved in a variety of projects, and can work with this developing service as we bring more projects on board. - Base salary starting at £37K, rising to £43K dependent on experience.

To be eligible for these roles, you must be an IAPT Qualified Psychological Wellbeing Practitioner (PGCert Low Intensity Psychological Interventions), with a minimum of 6 months experience within Primary Care Mental Health. BABCP or BPS registration is required.
