Job Description

Job Title: Primary Care Occupational Therapist (OT)

Responsible to: Practice Manager

Accountable to: Partners and Practice Management

Hours of employment: up to 28 hours per week

Salary: Approx. equivalent to AFC Band 7 (not AFC contract)

Job Summary:

This post requires an OT who is highly specialised and can work at an advanced level of practice within the Linden Medical Group, utilising their in-depth knowledge, clinical skills and experience at a senior level, in the management of patient’s occupational therapy conditions.

The post holder will provide intervention for the identified cohort of patients which includes the frail, elderly, care homes, palliative and complex patients from initial clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for. They will work collaboratively with the Practice’s and
PCN’s multi-disciplinary team (MDT) to meet the needs of patients and carers and support the delivery of policy and procedures.

The post holder will lead a team of Care Coordinators providing guidance, line management and support.

Person Specification

Education and Qualifications

Essential

BSc in Occupational Therapy under a training programme approved by the college of OTs

Health & Care Professions Council (HCPC) Registration

Able to operate at an advanced level of clinical practice

Evidence of appropriate continuing professional development and education to maintain up to date knowledge

Desirable

Independent prescriber

Pre-reg MSc in Occupational Therapy under a training programme

approved by the College of OTs

Experience

Essential

Experience of assessment and treatment delivery in long term conditions and active management

Experience of working with a multi-disciplinary team

Experience of implementing protocols and clinical guidelines

Experience of managing complex and changing conditions in patients

Experience of implementing change management

Experience of managing and developing a team

Experience of developing services

Desirable

Experience in a community or primary care setting
