Specialist Mental Health Services for Older People

The Trust is seeking a consultant old age psychiatrist to join the Specialist Mental Health Services for Older People

The Community Mental Health Services for older people in the trust consist of early memory diagnostic and support service (EMDASS), recovery and therapy service and crisis function as well as elements of integrated primary care services. The old age liaison services are provided by RAID teams based at East London hospitals.

East London is a popularly and informally defined part of London, capital of the United Kingdom. By most definitions, it is east of the ancient City of London and north of the River Thames. It broadly comprises the London boroughs of Barking and Dagenham, Hackney, Havering, Newham, Redbridge, Tower Hamlets and Waltham Forest.

From the iconic skyline of Canary Wharf to the parks and Victorian terraces of Hackney, East London's landscape tells a tale of rise and regeneration.

Anyone with an interest in multicultural London needs to visit the East End. There's standout ethnic cuisine, some interesting museums and galleries, excellent pubs, canal-side eating and drinking, some of London's hippest neighbourhoods, and the vast redeveloped expanse of Queen Elizabeth Olympic Park to explore.

In 2019, there were 1.1M residents in East London postcode area with an average age of 33.9 years. Population density was 10.0k residents per square kilometer. Population grew by 37.8% since 2002 and population average age increased by 0.9 years in the same period. Based on Census, there were 2.67 residents per household in 2011.

Department

This is a fixed term consultant old age psychiatrist post is a community based in the East quadrant of London (Over 65 population 378,000 approximately). The post includes covering Eastern half of the Specialist Mental Health Team for older People (SMHTOP) covering community area and crisis function team covering the entire North West sector and memory clinics.

Crisis function sub-team of the SMHTOP is based at The Health Facility & Well Being Centre in East London and works in the community like a Virtual Ward setting that allows for people to be cared for in their own home and will reduce unnecessary admissions to hospital and care homes.

This is in line with STP national integration agenda. The aim is to provide ‘joined up’, responsive and efficient services ensuring the patient receives the right treatment in right place at the right time.

The post holder will work closely with the Crisis Function team, SMHTOP, local GP practices and acute hospital liaison at a General Hospital (GH). Although the administrative base will be at the Health Facility, clinical work may be at other Trust locations, or other community destinations such as patient’s home or residential or nursing homes. The post holder may be required to attend team meetings at a location determined by the manager of the Team.

General Duties- guideline only

  • Assessment, diagnosis, and formulation of management plans for patients.
  • Make decisions in consultation with team members, patients and carers.
  • Flexibly review and re-assess patients in response to need and in accordance with the care pathway.

Know policy relevant to the role, follow it and support other members of the multidisciplinary team to do so also.

  • Regular or ad hoc supervision on clinical matters with member of the team. Maintain high clinical standards and develop the skills of the team. Lead a culture of high-quality clinical care. Explore new innovations in delivering care
  • Maintain good working relationships with primary and secondary care colleagues. Communicate clearly and responsively about patient care.
  • Meeting the requirements of emergency work, Community Treatment Orders, MHA assessments, and assessments of capacity. Maintain AC Status and attend relevant training.
  • Ensure monitoring for metabolic complications related to medication, lithium monitoring, and adherence to the clozapine protocols (if applicable).
  • Actively manage the outpatient clinical case load in accordance with pathway guidance. Operate efficient and safe patient review services such as drop-in clinics.
  • Provide timely clinical and management supervision to medical staff under their direction.
  • To ensure that junior medical staff working with the post holder operates within the parameters of the New Deal and are Working Time Directive compliant.
  • To undertake the administrative duties associated with the care of patients.
  • To record clinical activity accurately and comprehensively.
  • To participate in service and business planning activity for the locality and, as appropriate, for the whole mental health service.
  • To participate in annual appraisal for consultants.
  • To attend and participate in the academic programme of the Trust, including lectures and seminars as part of the internal CPD programme.
  • To maintain professional registration with the General Medical Council, Mental Health Act Section 12(2) approval, and to abide by professional codes of conduct.
  • To participate annually in a job plan review with the clinical manager, which will include consultation with a relevant manager in order to ensure that the post is developed to take into account changes in service configuration and delivery associated with modernisation.
  • To work with local managers and professional colleagues in ensuring the efficient running of services, and share with consultant colleagues in the medical contribution to management.
  • To comply with the Trust’s agreed policies, procedures, standing orders and financial instructions, and to take an active role in the financial management of the service and support the medical director and other managers in preparing plans for services.


Job Plan

Regular job planning meetings will be undertaken with the post holder to ensure that the appointee is remunerated for any additional activities undertaken and that the job plan accurately reflects the overall workload undertaken by the post holder.


The supporting professional activities (SPAs) allocation within the provisional job plan is an illustrative guide and will be finalised following individual agreement with successful candidates. Further discussion on supporting professional activities will depend on the Trust's requirements and the individual's particular expertise.

Salary


This is as described in the Medical and Dental Terms and Conditions, in line with the Consultant Contract (2003). The current full time salary scale (as of July 2020) ranges from £84,559 - £114,003 a year with eight thresholds.


The on call supplement is Category A and attracts a supplement of 5% of basic salary.

Office Accommodation

Office accommodation will be provided for the appointee with secretarial support.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the existing clinical team through, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

Teaching/Research

The consultant will be required to participate in programmes for teaching clinical students, training junior doctors and in clinical examinations. The post-holder’s contribution to teaching, training and research will be included in the regular job plan review. Teaching will take place during programmed activities and flexibly at other times.

  • To provide conditions for improved training opportunities in line with national and local recommendations arising out of GMC and related requirements
  • To supervise and manage the work of his/her junior medical staff and participate in the training and development of these staff to the standards required by the Royal Colleges.

Clinical Governance & Audit

The consultant will participate in clinical governance activities, including clinical audit, clinical effectiveness, risk management, quality improvement activities as required by the Trust, and external accrediting bodies. There are 6 half day clinical governance meetings per annum which substitute regular fixed sessions. There is a weekly consultant meeting before MDT on Friday to discuss any issues during the week and to plan for next week.

Personal & Professional Development

The post-holder will be required to keep himself/herself fully up-to-date with their relevant area of practice and be able to demonstrate this to the satisfaction of the Trust.

The Trust supports the requirements for continuing professional development as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities.

Medical revalidation is the process by which the General Medical Council (GMC) confirms the continuation of a doctor’s licence to practise in the UK, provides greater assurance to patients, the public, employers and other healthcare professionals that licensed doctors are up-to-date and fit to practise. It is a key component of a range of measures designed to improve the quality of care for patients.

The Trust supports Royal College guidance on provision of mentors for new consultants in line with GMC recommendations. All newly appointed consultant staff are provided with a list of locally trained mentors who are available for ongoing support as required.
