We are currently looking to expand our 24 hour- 7 days per week Response service and are seeking people with drive and enthusiasm to join the team. This is a fantastic opportunity to be a part of a service who provide an emergency 2 hour response to patients via referrals into our Urgent Community Response team and activations from Carecall pendants and telecare equipment. You will be working closely with our Urgent Community Response team and Carecall Service. Your role will be to attend properties identified by both services providing assistance in emergency situations and enabling customers to remain safely at home wherever possible. Our aim is to reach a patient who has fallen within a 2 hour period and in the majority of cases avoid a hospital admission
It does not matter whether you have previous experience of this role as a comprehensive training package will be provided, what is really important are your values and your attitude towards working with people who need support in a crisis and to offer them both professional and emotional reassurance.
Urgent Community Response in addition to our well established Care Call services are one of seven nationally acclaimed accelerator sites for NHS England’s Ageing Well programme. They deliver a response service to people experiencing a health or social care crisis. Our highly skilled team is made up of health and social care professionals working together to support people at home or in the community and in doing so contributing to the avoidance of a hospital admission and keeping people safe and well in their own homes.
Intermediate Care at Home where we provide a wide range of care, social work, rehabilitation, therapy, equipment and assistive technology services supporting people to live well and independently either at home or in the community. We take pride in our GOOD CQC registration for our care services and our reablement outcomes are strong with over 80% of people receiving our service remaining independent at home for three months after leaving our service.
Hospital Discharge Team and Intermediate Care Bed Bases where we support the safe and timely transfer from hospital into settings that best meet individual needs, providing care and rehabilitation to regain independence and undertake assessments for their long term needs.

- A good level of literacy and knowledge and the ability to learn new computer programmes.
- A current driving licence.
- The flexibility and willingness to work to a rota that includes days, nights, weekends and bank holidays .
- A competitive rate of pay with payment by shift not by call and enhancements for evening and weekend working
- A planned rolling rota (but with some flexibility within the service demands)
- Generous paid holiday allowance of 25 days per annum (rising to 30 days after 5 years)
- Use of a pool car (with fuel) during your shift to travel between calls
