Job Description

Job Title:

Complex Care Support Worker Wake Nights - Liverpool

*Must have immediate availability and tracheostomy experience within the last 6 months*

We are looking for a dedicated and compassionate Support Worker to support a 16 year old female with cerebral palsy and complex health care needs in her own home.

The role involves all aspects of personal care, manual handling, PEG care, tracheostomy care, stoma care, administration of medication and support with communication.

You will work as part of a care team alongside the family and a team of therapists to provide person centred and individualised care and rehabilitation. You will also be required to maintain high standards of cleanliness with the client and within the home environment.

The client has her own wheelchair adapted vehicle which you will be asked to drive to take her to various appointments. Full training will be provided.

Location:

The client’s residential home in Liverpool, but you may be required to work from other locations at the discretion of the company and with appropriate notice.

Pay:

£12.50 week day rate, £13.50 week night rate, £16.66 weekend rate.

Sleep in - £81 week night sleep, £99.96 weekend sleep (10pm - 8am)

Working Hours:

The client requires 24/7. Hours to be agreed with Manager. Rolling rota will be provided with occasional requirement for flexibility, and subject to change with reasonable notice.

Shift patterns vary. Days and sleep nights, including 24 hours.

3 x 12 hours waking nights 8pm - 8am urgently required

Safety:

Applicants must be willing to undergo appropriate screening. This includes checks with past employers and an Enhanced Disclosure via the DBS. *It is a criminal offence for people who are barred from working in regulated activity to apply for roles that require them to work unsupervised with that particular group (i.e. adults at risk, children or both)

Reports to:

Case Manager / Care Co-ordinator

Job Overview: (Note - In addition to these functions, employees are required to carry out such duties as may reasonably be required)

· To provide care in accordance with best practice and legislative requirements, reflecting policies and procedures and agreed standards under the direction of the Case Manager / Care Co-ordinator

· To support and enable service users to maintain skills and personal interests while delivering person-centred care unique to each individual

· To maintain skills at a current level and undertake such training and development as required, from time-to-time, to maintain and progress

Responsibilities and Duties of the Job:

The Complex Care Support Worker’s responsibilities include but are not limited to the following:

· Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing

· To contribute to the efficient running of the service

· Support service users to maintain their relationships and connections with the local community

· Ensure care plans and other information about how to support service users are followed

· Be responsible for informing the Case Manager / Care Co-ordinator of any changes in the needs of service users

· Be responsible for promoting and safeguarding the welfare of those individuals they support

Working with Others:

· Develop effective working relationships with other employees within the service

· Work in cooperation with members of the multidisciplinary teams to maximise opportunities for service users

· If desired by the service user, maintain and develop relationships with their family, friends and other people important in their life

Person Specification

Qualifications

Essential

- Good English - written and verbal

- Care Certificate or equivalent

- Level 2/3 Health and Social Care Diploma or willingness to work towards

- Evidence of continued professional development

Knowledge

Essential

- Understanding of cerebral palsy

- Knowledge of CQC fundamental standards and key lines of enquiry

Experience

Essential

- 1 year minimum experience in health and social care

- 6 months minimum experience in working with people with neurological impairment

- 6 months minimum experience in complex care including tracheostomy carep

Specific Requirement for Skills

Proficient Written Skills

Maintain all care plans/care records in accordance with the service’s policy

Leadership Skills

Ability to induct and orientate new employees to the job role and service

Provide service users with support/assistance as they accomplish daily tasks, including bathing, eating, dressing, grooming and using the bathroom

Communicate any problems, concerns or changes to service users’ family members as needed

Communication Skills

Carers must build a rapport with service users by establishing personal connections and showing an interest in their lives

Carers must be able to communicate effectively with service users

Carers must have the communication skills to inform colleagues, management and professionals about the needs of service users

Problem-solving Skills

Carers need to be able to adapt to and address situations quickly

Plan, develop, implement and assess approaches to promote health and wellbeing, whilst recognising and reporting situations where there might be a need for protection
