Are you a registered nurse, OT, social worker or AHP looking for a change? Would you like to support service users to build a meaningful life in the community? Does the idea of working in a recovery focussed way with a strong focus on coproduction appeal to you? Do you have experience of working with individuals with complex needs? If this sounds like you then The Community Rehabilitation Enhanced Support Team (CREST) would love to receive your application.

We are looking for a dynamic, enthusiastic, and motivated individuals with an interest in working in a psychologically and trauma informed way to offer rehabilitation focussed interventions to individuals who have spent long periods of time in hospital. CREST is part of the integrated care systems approach to working differently to improve outcomes for service users, and CREST is the first of these workstreams to become operational.

Travel will be required across the West Yorkshire Area, with bases in Halifax and Leeds. There will also be some travel to out of area hospitals outside of the West Yorkshire Footprint as part of assessment and engagement.

CREST support individuals within complex rehabilitation to address barriers to discharge, assess their rehabilitation needs and identify how these could be successfully supported in a least restrictive environment. The varied needs of the service users within CREST’s caseload and their long history of support within inpatient environments mean that CREST need to take an innovative approach to support to ensure successful outcomes.

CREST work closely as an MDT which compromises Clinical Nurse Specialist in Addictions, Family Engagement Worker, Medics, Nurses, Occupational Therapists, Peer Recovery and Psychology Team. This diverse MDT ensures that service users can access a range of support from within the team, with a focus on co-production and creating supportive networks. In addition to being part of a supportive team with a range of specialist skills, there is also a strong focus on multi-agency working, with many of the caseload being supported by private hospital providers, supported living and a range of statutory and non-statutory organisations.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. As a teaching trust with strong links to local universities, we are a centre of excellence for teaching, research and development.

There are many benefits of working for LYPFT including fantastic employee wellbeing support, 27 - 33 days annual leave, flexible working and remote working arrangements, the NHS Pension Scheme (LYPFT pays 20.6% into your scheme each year), coaching, support and opportunities for career development and training and education support alongside a range of exclusive discounts and payment schemes including for cars, extra leave days and more organisations such as Blue Light Card, Health Service Discounts and NHS Discount Offers.

We also have an incredible bank department, offering variety of roles in nursing, allied health professions, healthcare support worker and administration clerical. Permanent employees are automatically added to bank. Bank-only workers can choose the hours they want to work and will have the opportunity to gain additional experience, keep skills up to date, develop new ones and earn extra money without having to commit to a permanent role. For an informal discussion or more information please contact the [email protected]

We are currently recruiting for a care coordinator as part of our regional service covering Bradford, Calderdale, Kirklees, Leeds and Wakefield and supporting service users reconnect with the community after a stay in complex rehabilitation. The role will involve coordinating the care of individuals in line with changes in national direction of the Care Programme Approach and offering evidence-based interventions to address core rehabilitation needs and manage risk within a community setting.

Our commitment to you if successful is that you will be offered strong support structures including reflective practice and clinical supervision, be supported to develop new skills, psychologically informed training and opportunities to be part of evaluation and development of the service.

We recognise the importance of ensuring that our services are reflective of the communities we serve and champion diversity and inclusion. As part of a wider strategy to improve healthcare career opportunities for all, we would particularly welcome applicants from Black and Minority Ethnic (BAME) backgrounds. We are committed to implementing the NHS Workforce Race Equality Standards and welcome staff members from all racial backgrounds who commit to these standards.

CREST was established as part of the complex rehabilitation project with the aim to support repatriation from locked rehabilitation placements and a strong focus on empowering service users and carers in being actively involved their recovery journey. The service currently operates 8am-8pm during the week, and 9-5 over on a weekend.

The Complex Rehabilitation project is part of the West Yorkshire Integrated Care System (WYICS) Programme for Mental Health and Learning Disability. Consultation with service users, carer’s, commissioners and mental health providers has highlighted the service user experience and challenges for people placed within mental health complex (locked) rehabilitation placements from West Yorkshire. Many people have experienced long lengths of stay in locked hospital placements, are disconnected from a community. Service users have shared very powerful experiences with us, describing a community model as “not just about keeping me safe – it’s about building a life worth living”.

In order to meet the aims identified as part of the consultation project CREST has been working on developing a clinical model which supports individuals on their pathway from locked rehabilitation. This psychologically informed model focusses on placing the service user and their loved ones at the heart of what we do, assessing the need and identifying interventions that are needed to support and empower individuals on their recovery journey.

If successful you will be responsible for a small caseload in line with the care programme approach of service users supported by the wider multi-disciplinary team and on a day to day basis work within the framework of the team approach. You must have good working knowledge of the principles of recovery and practice clinically in a way that promotes therapeutic optimism and hope with service users.

Assessment also forms a key part of CREST’s role, with referrals coming from case managers across the West Yorkshire region for individuals who are within, or on occasion being considered for, out of area rehabilitation with CRESTs focus being on identifying their current needs, barriers to establishing a discharge pathway and making recommendations on interventions that would support them in their discharge pathway.

You would also be required to coordinate the delivery of care within the role of duty worker, ensuring the effective prioritisation of care and resources across the shift. There will be the need for close liaison with service users, carers and loved ones, other service providers and the wider MDT to ensure high-quality care is delivered.

If working in an innovative recovery focused team is something that you like the sound of and you’d like additional information then please do get in contact to find out more about the role and CREST.
