An exciting opportunity to be part of a dynamic team at the heart of hospital discharge has arisen. The post is for a Band 5 Social Worker working within the Discharge to Assess team covering the South West. Multi-professional working is a key skill required as you will work alongside therapists, care providers and others such as Mental Health services to support people to return home and regain their independence. The interventions are short term and the work is of a fast pace but rewarding when the outcome is that the person is able to live as they choose in their own home.

As a Band 5 Social worker you will undertake the more complex interventions so will require good knowledge of The Mental Capacity Act and The Care Act. You will also be required to complete Safeguarding investigations where needed.
The Social Work Learning Academy offer a wide and varied range of training and there is the opportunity to complete your ASYE if required
As you will be visiting people in their own homes it is essential that you are a car owner and able to drive.
The post will include some weekend working on a rota basis.
