Would you like to part of a wonderful Community Rehabilitation team based in West Mendip Hospital in Glastonbury that provides intensive rehab in patients homes?

Would you like to have the opportunity to join us as a band 3 or 4 rehab assistant, depending on experience, with possible progression onto our physiotherapy apprenticeship provided with our organisation?

1. Support individuals to remain independent in the community through strength based approaches and the use of patient centred goals. Enablers, carers and relatives need to be considered in this process.
2. Where appropriate, facilitate eligible social care needs, through assessment, alongside other multi-disciplinary professionals in line with The Care Act 2014.
3. Review and monitor plans with individuals and their support networks, making recommendations for adjustment as considered appropriate.
4. Inform, advise and support individuals and their support networks in accessing a variety of community services.
5. Protect individuals and their support networks from harm or abuse as far as possible, consulting with a professionally qualified member of staff or Manager whenever there are concerns in line with the Safeguarding requirements of The Care Act 2014.
6. Maintain and update appropriate administrative/IT records on individuals and their support networks as required by the service guidelines and provide timely information for evaluation updates.
7. Update and maintain awareness of new equipment and technology as part of continuing professional development.
8. As a fully autonomous practitioner take full responsibility, and legal accountability for, individual clinical decisions and actions
9. Plan and prioritise their own clinical workload, ensuring a prompt and efficient service using clinical judgement and service guidelines
10. Ensure all individuals and their support networks understand the ‘home first, home is best ’principles and process. Ensure the individual has given valid and informed consent.
11. Take into account an holistic/person-centred approach of the individual, in order to refer/signpost, to other services as necessary
12. When identifying resources, such as adaptive equipment, care and support; consider finance, eligibility and individual need.
13. Formulate appropriate person-centred goals based on individual need
14. Assess and evaluate the effects of the rehabilitation plan over time, altering the treatment approach as necessary and communicating this with individuals and providers
15. Contribute and facilitate where appropriate to the discharge planning process when individuals are discharged from the service.
16. To support patients within personal and domestic activities of daily living in relation to their goals and intervention plans.
