Job Description
We are seeking motivated, forward thinking, passionate individuals to join our reablement team supporting up to 37 individuals progress through their journey back to their long term care destination.
We have a team of professionals made up of health care assistants, qualified nurses and care co-ordinators who work in partnership with City Care to provide reablement support to those who have experienced a hospital stay and who are not ready to return home but who are medically fit for discharge from hospital. During their stay in our service they will receive support from physiotherapists and OT's with the goal of returning mobility and independence to their original base line.
This position is similar to those of Health Care Assistants in a hospital setting. You will support admissions and discharges daily, promote and encourage independence and ensure that during your duty, citizens in your care have their individual needs met around nutrition and hydration, tissue viability and personal care.
We currently provide support for 12 nursing needs citizens, 6 post stroke reablement, 14 enhanced care and 5 interim social wait beds.
Enhanced care means those who may display behaviours that others could find challenging. We do not actively accept citizens known to be physically aggressive however it is important to consider that those in enhanced care placements are often living with mental health or symptoms of advanced dementia.

There are opportunities within our service should you wish to take them including training at levels 2 and 3 with qualifications in health and social care. We can also support those of you who perhaps would like to become Nurse Associates and those who would like to become qualified nurses.
You will be supporting individuals with a range of underlying health conditions and needs. There are opportunities to work with those living with mental health conditions, physical complexities and at times, some behaviours that some may find more challenging. The citizens we support are ever changing, but the high level of service we deliver remains consistent. We go by the Moto that "the standard you walk by is the standard you accept" and our standards are high as you would expect from anyone looking after someone's loved one.
We have day time positions to fill and can offer part time or full time positions. We work across a 7 day rota. We are not able to offer 'set shifts' but as an employer who champions equality, we shall always try to support individual circumstances.
We have early shifts 07.00-14.00
We have late shifts 14.00-20.30
We have long days 07.00-20.30
