Job summary

An exciting opportunity for an experienced Pharmacy Technician to join our expanding team working with Horsham Collaborative Primary Care Network (PCN). The clinical pharmacy team is developing and we are looking for a motivated individual who is flexible to respond to the developing nature of the role and the needs and priorities of the service.

The successful candidate will work across four practices developing this role closely supported by the Lead Clinical Pharmacist and GPs. They will be part of a group of Clinical Pharmacists and technicians working under the supervision and mentoring of a Lead Clinical Pharmacist.

The role will suit a forward-thinking and enthusiastic pharmacy technician who is able to work collaboratively across the team. If you are adaptable, self-motivated and organised, possessing the necessary qualities and skills for this post, we look forward to hearing from you.

The post holder will work as part of a multidisciplinary team to support processes for structured medication reviews, high risk drug monitoring, management of medicines on transfer of care and systems for safer prescribing. They will work directly with patients and support the delivery of clinical care.

Main duties

To work with our team of Clinical Pharmacists and technicians to support the safe, accurate and timely prescribing of medication to patients in our Primary Care Network. Provide technical and administrative support to the Clinical Pharmacists and Clinicians. Support the primary care team with audits, guidance and respond to queries around medication from patients; including face to face and telephone support to patients, primary and community staff.

Horsham Collabrative PCN comprising of the following practices:

We currently have a PCN population of approximately 32,000 patients. The team is headed by our Clinical Director Dr Matt Greenwood and Lead Clinical Pharmacist Yasmin Hoque supported by our experienced management and administration team. We have experienced clinical pharmacists, technicians and paramedics in our team.

We provide access and support the CPPE Primary Care pathway course.

Job description

Job responsibilities

The pharmacy technician will work with the Lead Clinical Pharmacist, Pharmacists, Pharmacy Technician and Clinicians to support and develop the medicines management and prescribing function of our member practices.

To provide advice and guidance to patients regarding medication optimisation and lifestyle e.g. inhaler techniques.

To provide medication review services to patients via clinics in practices, domiciliary visits and in residential and nursing homes and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.

To support the achievement of the Primary Care Networks prescribing Quality and Improvement targets; working in collaboration with Practice teams.

To assist the Lead Clinical Pharmacist with data analysis of prescribing behaviour in line with NICE guidance and local clinical commissioning directives.

To assist with the development and delivery of medicine audits.

Conduct medication reconciliation following discharge from hospital, clinic visits and other transfers of care (e.g. new patients, care home residents), to ensure that medication changes are safely implemented in a timely fashion.

To link with hospital and community pharmacy colleagues to address discrepancies, supply issues and queries within competency.

To undertake regular audits using different tools, for example to identify priority patients for medication review.

To support the safe and effective repeat prescribing of high-risk medicines ensuring that regular monitoring as per local/shared care guidance is taking place.

To promote cost effective, safe, evidence-based prescribing in accordance with local formulary, medication review services to patients via clinics in PCN practices and care homes and to deliver plans that maximise cost-effective prescribing and improve the quality of patient care.

To assist in the monitoring of prescribing expenditure at individual practice level and propose changes to ensure containment within identified resource.

Attend local meetings seeking to improve access to medicines or repeat prescribing processes.

To provide Link/liaise with local strategic groups and stakeholders, e.g. CCG, community pharmacy and hospital teams, to ensure work is aligned with local health and wellbeing priorities.

Establish positive working relationships and effective communication with network practices, integrated care health and care teams, and a range of local voluntary sector and community organisations.

Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.

To use highly developed knowledge and skills to provide high standards of patient centred care.

Contribute to the development of the service, e.g. developing service pathways, paperwork and sharing good practice across network practices.

Participate in data collection data for audit purposes and be responsible for monitoring progress against key performance targets using EMIS Web.

To attend and participate in practice clinical meetings and other multi-disciplinary meetings where necessary.

Contribute to the achievement of team objectives.
