We are currently seeking to recruit to a number of permanent social work posts within our Countywide Hospital Discharge service. Posts will be based at Chesterfield Royal Hospital Integrated Discharge Team and the Community based Discharge Assessment and Review Team but there is an expectation to provide support to other hospitals when required.
The Chesterfield Royal Hospital Discharge Team primarily provides short term assessment and support to facilitate safe transfers of care, or to prevent unnecessary admission to hospital to enable people to continue living in their own home as long as possible. Post holders will be expected to work within tight discharge timescales and therefore will be of interest to applicants who enjoy working within a busy integrated Multi- Disciplinary setting.
The Discharge Assessment and Review Team is a community based, county wide team, with largely home based working arrangements, and social workers will provide assessment and support for people who have been discharge onto the various Discharge to assess pathways. You will support people to achieve their maximum levels of independence after a stay in hospital. This means that the work will be varied and mainly focus on longer term assessments and provide person centred planning for eligible needs, including both formal and informal services.
You will work in a strengths-based way, recognising people's assets and strengths and build networks and positive relationships with acute and Community Trust Health Colleagues and Agencies.
We expect the applicant to be confident in using Care Act 2014 and have a good understanding of Making Safeguarding Personal and the Mental Capacity Act.
Successful applicants will need to work in a team setting, cover part of a duty rota system and be expected to manage a busy, complex caseload. Good communication with other agencies is essential.

Experience of working in Adult care , is beneficial but not essential. You will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people utilising a Home First approach.
The post is for a 5 from 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. Prior notice will be given in advance if this changes.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
