Overview

This post is based in the Infection Science Department at the hospital. The Department of Infection Science consists of Microbiology, Virology and Immunology, all centralised at hospital site. Consultant microbiologists will be required to work across site on a rotational basis at the General Hospital in the region.

The successful applicant will be encouraged to support the development of a joint microbiology and virology consultant team. The virology service offers viral serology and molecular tests, and provides services to other trusts within the UK and Ireland. The immunology and virology laboratories are fully integrated with shared staff and platforms but with separate clinical leadership arrangements. The immunology laboratory is a national reference centre for Bird Breeders and Farmers Lung.

Wiltshire is a county in South West England with an area of 3,485 km2 (1,346 square miles). It is landlocked and borders the counties of Dorset, Somerset, Hampshire, Gloucestershire, Oxfordshire and Berkshire.

Wiltshire is characterised by its high downland and wide valleys. Salisbury Plain is noted for being the location of the Stonehenge and Avebury stone circles and other ancient landmarks, and as a training area for the British Army. The city of Salisbury is notable for its medieval cathedral. Important country houses open to the public include Longleat, near Warminster, and the National Trust's Stourhead, near Mere. It is a UNESCO Cultural and World Heritage site.

In 2019, there were 722k residents in Wiltshire county with an average age of 41.7 years. Population density was 207 residents per square kilometer. Population grew by 16.4% since 2002 and population average age increased by 2.9 years in the same period. Based on Census, there were 2.39 residents per household in 2011.

Department
The Laboratories

The Department of Infection Sciences is accredited to ISO: 15189 standards. The laboratory consists of Microbiology, Virology and Immunology, all of which participate in appropriate UK National Quality Assessment Schemes. The laboratories are currently registered with the Health Professions Council as a training laboratory for Biomedical Scientists.

A comprehensive Microbiology, Virology and Immunology Service, centralised at the hospital is offered to the hospitals, community services and primary care within Wiltshire. There are no Infection Sciences laboratory at General Hospital.

The laboratory information management system relies on Telepath for request entry, work scheduling, result entry and report printing. This is integrated with the Sunquest ICE order comms system for ward ordering of tests and electronic reporting.

The Department of Infection Sciences has an annual budget of £3,255,088. The budget is administrated by the Operational Manager and is held by the Head of Department.

The diagnostic workload for 2019 was approximately 210,000 tests performed in Microbiology, 125,000 tests performed in Virology, and approximately 118,000 tests performed in Immunology. For Microbiology about 60% of the specimen load comes from the Trust, and 40% from Primary Care. For Virology and Immunology about 30% of the specimens are from the acute trust, 40% from primary care, and the remainder is from other trusts or referred from other laboratories.

There is a category 3 labs plus a dedicated enteric lab. The Microbiology lab is well equipped with a BD BACTEC FX blood culture system, DS2 ELISA system, Don Whitley Anaerobic cabinet, walk-in Hot and Cold rooms, IRIS automated urine analyser, Cepheid GeneXpert System, and MAST Urisystem.

The Virology service offers viral serology and some viral detection to Wiltshire. This Laboratory also provides some tests to other trusts within the UK and Ireland. A variety of PCR tests including Influenza, VZV, HSV, and, Norovirus testing is carried out within the department. The laboratory also receives samples for Chlamydia and gonococcal screening by PCR for Wiltshire CCG.

Immunology is also based in this laboratory providing a wide range of tests, which complements Virology and other areas of Microbiology. This service has excellent relationship with the Immunology Laboratory in Bristol and the laboratory in Swindon. Within this section is a reference centre for Bird Breeders and Farmers Lung.

The Trust refers samples for Mycobacteria to a specialist centre.

The department is part of the Pathology Directorate.

Essential:

  • FRCPath or equivalent
  • Full and Specialist registration (and with a licence to practise) with the General Medical Council (GMC), or be eligible for registration within six months of interview.
  • Applicants that are UK trained, must ALSO be a holder of a Certificate of Completion of Training (CCT), or be within six months of award of CCT by date of interview.
  • Applicants that are non-UK trained, will be required to show evidence of equivalence to the UK CCT


Experience & Knowledge


  • High standard of clinical skill and expertise in clinical microbiology
  • Clinical Governance
  • Ability to offer expert clinical opinion on range of clinical microbiology problems both emergency and elective and in primary care.
  • Ability to take full and independent responsibility for liaising with clinical staff for the clinical care of patients with infections.
  • Experience in infection prevention and control


Job Plan

Regular job planning meetings will be undertaken with the post holder to ensure that the appointee is remunerated for any additional activities undertaken and that the job plan accurately reflects the overall workload undertaken by the post holder.


The supporting professional activities (SPAs) allocation within the provisional job plan is an illustrative guide and will be finalised following individual agreement with successful candidates. Further discussion on supporting professional activities will depend on the Trust's requirements and the individual's particular expertise.

Out of Hours Responsibilities including on-call commitments


There is a 1 in 4 on call commitment with this post when fully staffed. This commitment may temporarily increase during periods of absence of other consultants


Salary


This is as described in the Medical and Dental Terms and Conditions, in line with the Consultant Contract (2003). The current full time salary scale (as of July 2020) ranges from £84,559 - £114,003 a year with eight thresholds.


The on call supplement is Category A and attracts a supplement of 5% of basic salary.

Office Accommodation

Office accommodation will be provided for the appointee with secretarial support.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the existing clinical team through, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

Teaching/Research

The consultant will be required to participate in programmes for teaching clinical students, training junior doctors and in clinical examinations. The post-holder’s contribution to teaching, training and research will be included in the regular job plan review. Teaching will take place during programmed activities and flexibly at other times.

  • To provide conditions for improved training opportunities in line with national and local recommendations arising out of GMC and related requirements
  • To supervise and manage the work of his/her junior medical staff and participate in the training and development of these staff to the standards required by the Royal Colleges.

Clinical Governance & Audit

The consultant will participate in clinical governance activities, including clinical audit, clinical effectiveness, risk management, quality improvement activities as required by the Trust, and external accrediting bodies. There are 6 half day clinical governance meetings per annum which substitute regular fixed sessions. There is a weekly consultant meeting before MDT on Friday to discuss any issues during the week and to plan for next week.

Personal & Professional Development

The post-holder will be required to keep himself/herself fully up-to-date with their relevant area of practice and be able to demonstrate this to the satisfaction of the Trust.

The Trust supports the requirements for continuing professional development as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities.

Medical revalidation is the process by which the General Medical Council (GMC) confirms the continuation of a doctor’s licence to practise in the UK, provides greater assurance to patients, the public, employers and other healthcare professionals that licensed doctors are up-to-date and fit to practise. It is a key component of a range of measures designed to improve the quality of care for patients.

The Trust supports Royal College guidance on provision of mentors for new consultants in line with GMC recommendations. All newly appointed consultant staff are provided with a list of locally trained mentors who are available for ongoing support as required.
