An exciting opportunity has arisen for two Assistant Nurse Practitioners to work within the Long Term Care Team.
The post holder will be responsible for reviewing patient's eligibility for NHS Funded Nursing Care and NHS Continuing Health Care in care homes and community settings, where appropriate, in line with The National Framework for Continuing NHS Healthcare Wales 2021.
