We are seeking enthusiastic, motivated and caring individuals to join our Reablement Team providing support to people living in their own homes.
The Reablement Service are a dedicated team, whose main goal is to enrich the lives of those living at home to ensure they maintain their independence and healthy living.
The aim of the service is to enable and empower people to undertake their own daily living tasks, with the support of small aids, guidance and encouragement.
Valid driving licence and use of own vehicle is required as he role involves lone working in the community.
Your responsibilities will include:
- Using goal plans and observational skills to participate in the reablement of people.
- To monitor and feedback any outcomes/concerns/progress/deterioration regarding people’s set goals.
- Support people to self-medicate, or to assist or administer medication.
- Maintain accurate records in the person’s home including the completion of goal progression, medication records and any other required paperwork.
- Promote and facilitate access to community resources e.g. public transport, local shops, day services etc.
- Support people to undertake practical everyday tasks to support and meet their outcomes and regain independence.
Valid driving licence and use of own vehicle is required as he role involves lone working in the community.
We offer full free training, free DBS checks, a competitive salary and we also pay for travel time between clients and mileage at 45p per mile.
